Violation descriptions and comments

All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. Critical violation: found black slimy substance on various surfaces of the ice machine reservoir making contact with the ice cubes inside said machine. Instructed to clean and sanitize affected areas and maintain. Citation issued.
Adequate and convenient hand washing facilities shall be provided for all employees. Critical violation: Found no hand soap and paper towels at the exposed hand wash station and in both men's and women's washrooms. Instructed to provide. Citation issued.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Serious violation: Found twenty (20) rodent droppings under and by side of cooking equipment. Instructed to remove, clean and sanitize said area. Advised to contact pest control operator for service. Citation issued.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Minor violation: found cans of beverages stored in a potable ice bin that is used to serve cubed ice to customers. Instructed to remove.
Smoking shall be prohibited in all public places except in designated smoking areas. Serious violation: Found an ashtray with four (4) cigarette butts sitting on top of a box in the food/beer utility room. Instructed that no smoking take place anywhere in a public place. First offense. Citation issued.

All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. LIQUOR STORAGE ROOM CLUTTERED. MUST ELEVATE ALL ITEMS 6' OFF FLOOR MUST BE ABLE TO CLEAN FLOOR UNDER AND AROUND ALL ITEMS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. THE FOLLOWING NOT CLEAN- BAR GUN HOLDERS, REACH IN PREP COOLERS DOOR RAILS WITH FOOD DEBRIS AND SLIGHT LIME BUILD UP INSIDE ICE MACHINE. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED ACCUMULATED DUST/DIRT AND BOTTLE CAPS BUILD UP ON FLOOR THROUGHOUT BAR. MUST DETAIL CLEAN FLOOR THROUGHOUT BAR AND MAINTAIN.
None of the operations connected with food establishments shall be conducted in any room used as living or sleeping quarters. MENS WASHROOM DOOR NOT SELF CLOSING. MUST PROVIDE AND INSTALL SELF CLOSING DEVICE FOR WASHROOM DOOR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PREP AREA HANDWASH BOWL DRAINING SLOWLY. MUST REPAIR AND MAINTAIN SINK.
