Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO PROVIDE WORKING DRAIN STOPPERS AT 3 COMPARTMENT SINK.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO REMOVE ALL UNNECESSARY ARTICLES STORED IN REAR STORAGE/BOILER ROOM TO PREVENT RODENT AND INSECT HARBORAGE.----- ITEMS MUST BE STORED 6' OFF THE FLOOR FOR EASY ACCESS ON CLEANING.---- MUST PROVIDE PROPER RAISED SHELVINGS.

Walls not maintained in rear storage room. Instructed to seal and/or paint walls. Gaps noted between floor and lower wallbase in storage room. Instructed to install coving. Ventilation cover in rear corner above walk-in freezer not cleaned. Instructed to detail clean.
Supplies in rear storage room not elevate off floor. Instructed to elevate supplies 6 inches or higher and away from wall.
