Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. The sink sprayer hangs into the sink slightly, the sprayer was tethered up at this time.
The walls and ceilings shall be in good repair and easily cleaned. Replace stained ceiling tile in the 'SUNSHINE' class room.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Instructed cook to remove some of the juice bottles blocking the 2-door cooler circulation fans.

The walls and ceilings shall be in good repair and easily cleaned. REMOVE PEELING PAINT FROM STALL WALL OF CHILDRENS WASHROOM ON SECOND FLOOR AND REPAINT. REPLACE TWO MISSING WALL TILE AT LOWER WALL OF ADULT WASHROOM ON SECOND FLOOR.

NO COLD RUNNING WATER AT ONE SINK IN GIRLS WASHROOM IN BASEMENT; PROVIDE.

THE BASEMENT REFRIGERATOR HAS A BROKEN THERMOMETER, REPLACE THE THERMOMETER.
THE WINDOW FRAME IN THE SOUTH 1ST FLOOR CLASSROOM IS DAMAGED AND PEELING, REPAIR THE WALL. THE DOLPHIN ROOM BASEBOARDS ARE DAMAGED IN THE NORTH EAST CORNER, REPAIR THE BASEBOARDS.
THE OFFICE BATHROOM WINDOW HAS A WIRE IN THE WAY AND WILL NOT CLOSE PROPERLY, REMOVE WIRE TO KEEP INSECTS FROM ENTERING OR USE A SCREEN.
RE-PRINT THE CONTENTS OF THE SANITIZER BOTTLES WHERE THEY HAVE WORN OFF.
