Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST PROVIDE BACKFLOW DEVICE AT MOP SINK IN BASEMENT.
The walls and ceilings shall be in good repair and easily cleaned. MUST SEAL GAP BETWEEN 2-WALLS AT CORNER OF 3-COMP SINK.

Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.All clean single service utensils must be stored with handles up.

A splashguard is needed at the exposed handsink at kitchen area,replace broken gaskets at the rear 2 door cooler.
Replace burnt out light bulbs,detail clean,maintain light shields over food prep area.
Ice scoops must be stored in clean dry place,wiping cloths must be stored,maintained in a sanitizing solution.
Detail clean,maintain hood above cooking equipments,remove dusts accumulations on ventilation covers in toilet rooms.
