The walls and ceilings shall be in good repair and easily cleaned. Must clean walls aroud/behind dishmachine and maintain.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. West walk in cooler door rubber gasket ripped. Must repair/replace damaged gasket.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed accumilated dust build up on prep coolers compressors. Must clean and maintain.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Basement floor not clean under and around unused chairs and along walls. Must keep floor clean and maintain.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed food build up inside unused reach in cooler and dust build up on rear of dairy walk in cooler compressor. Must clean and maintain all unclean equipment.
The walls and ceilings shall be in good repair and easily cleaned. Must detail clean all unclean walls throughout dry storage room.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Dishmachine sprayer nozzle in poor repair. Must repair/reattach nozzle to sprayer.

LIGHT SHIELDS MISSING AT THE DISHROOM AREA-MUST REPLACE. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities.
(7-38-005A) IMPROPER FOOD STORAGE. FOUND A PAN OF PAR-COOKED CHICKEN STORED ON THE VERY BOTTOM PART OF A WHEELED CART (INSIDE WALK-IN COOLER), BELOW THE TRAYS OF RAW FISH & RAW MEATS. INSTRUCTED TO STORE ALL COOKED FOOD ITEMS ABOVE THE RAW FOOD PRODUCTS. All food should be properly protected from contamination during storage, preparation, display, service, and transportation.
ALL FOODS STORED IN COOLERS MUST BE DATED & LABELED, INCLUDING BULK FOOD CONTAINERS. All food not stored in the original container shall be stored in properly labeled containers.
CUTTING BOARDS WITH DEEP/DARK GROOVES MUST BE REPLACED. ICE BIN LIDS NEEDED AT THE BAR-MUST PROVIDE. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
CONDENSERS, FAN GUARDS, COOLER DOOR GASKETS, SODA GUN HOLDERS, BOTTOM SHELF OF PREP COUNTER AT COOK'S LINE MUST BE FREE OF HEAVY DUST BUILD-UP OR FOOD DEBRIS/STAINS. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
FLOORS AROUND LEGS OF EQUIPMENT UNDER THE BAR SINKS, COOKING EQUIPMENT, INSIDE WALK-IN COOLER-MUST BE KEPT CLEAN. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
FAUCET SPRAYER AT THE SCRAPING SINK OF THE DISHMACHINE MUST BE REPLACED (IN POOR REPAIR).
