Observation: Food employees have not been informed in a verifiable manner of their responsibility to report, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Employees shall read, understand, and sign the FOOD EMPLOYEE REPORTING AGREEMENT FORM. The manager/owner made copies and prepared form for employees during the inspection.

Observation: The sanitizer for the final rinse of the dish machine was weak. This violation was corrected at the time of this inspection by changing the sanitizer and contacting the service company.

Observation: The bucket of sanitizer for the dish machine was empty and no sanitizer concentration was detected in the final rinse---The violation was corrected-on-site by changing the bucket and retesting the sanitizer. Reminded the manager about the need to routinely monitor the dish machine concentration and bucket levels.

Observation: An employee was eating in the food preparation area and had a Pop-tart setting on the prep table cooler---The Pop-tart was discarded and the manager instructed the employee about the proper location for eating.
