- R/*4.4-QAC sanitizer in spray bottle in back storage room was over 500 ppm. Maintain sanitizer per manufacturer recommendations or 200 ppm to avoid leaving a toxic residue on food contact surfaces. Dilute sanitizer to prevent toxic residue.
- C/*3.1 Two cans of Campbell's Spaghetti O's were deeply dented. Food packages shall be in good condition to protect the integrity of the contents. Items were pulled off shelf during this inspection.
- C/6.2 No handwashing signs visible in the restrooms or at the back room hand wash sink. Provide a sign at handwashing sinks notifying food employees to wash their hands. Signs were provided during this inspection.
- The following food items were beyond their sell by dates on the retail shelves:
- (4) boxes of Oat Revolution, Apples and Cinnamon, dated 3/9/12
- (1) box of Kellogg's Corn Pops dated 5/1/12
- (4) pints of Dairymens skim milk, 2 dated 5/18 and 2 dated 5/21
- Check and rotate stock on a regular basis. Items were pulled from retail shelves during inspection.

- 3.4- Foods on the roller grill were holding at improper temperatures including cheeseburger dog 90°F, hotdog 131°F and 125°F, and chicken ranch dog 105°F. Reportedly these foods were still cooking. No sign indicated foods were still cooking. Signs were put up during this inspection. Discussed lunch sandwiches which are not microwave prior to placing under the heat lamp (chicken sandwich 49°F). A time was indicated on the lunch sandwiches. Manager does not wish to change risk level to a level 4 and use time in lieu of temperature for sandwiches. Procedure will be changed and sandwiches will be held at 135°F and above and microwave prior to placing under the heat lamp. Breakfast sandwich at 155°F. Maintain potentially hazardous foods hot at 135°F and above.
- *3.4- Dates are not being changed on the containers in the reach-in cooler for the roller grill. Use by dates were 10/6, 10/12. and
- 10/13. Ready-to-eat potentially hazardous foods shall be date marked when opened and consumed within seven days.
- *4.4- QAC sanitizer in spray bottle under the coffee sink was over 500 ppm. Maintain sanitizer per manufacturer recommendations or 200 ppm to avoid leaving a toxic residue on food contact surfaces.
- *6.2- No paper towels at the back room hand wash sink. Provide an adequate supply of paper towels to facilitate hand washing.
- 6.4- Floor in back storage room is dirty. Clean the floor.
- Lucky Charms was removed from shelving with a sell by date of September 25, 2011. Please check and rotate stock on a regular basis.

- 7.0- Spray bottles were mislabeled. Degreaser labeled as roller cleaner and sanitizer in bathroom cleaner bottle. Properly label toxic chemicals to avoid confusion or misuse.
- 4.5- Nestle freezer has ice accumulation. Defrost the freezer.
- Notes: Foods were recently placed on grill roller and not at 135°F. Reportedly brackets to hold "not ready" signs are on order.
- Foods past sell by date include but not limited to: Keebler graham crackers 9/05/10, Apple Jacks 8/12/10, Fruit Loops 8/25/10 and
- 6/22/10. Check and rotate stock regularly.
