  • 3.4 -White cheese stack in walk-in cooler was date marked with 5/23. Reportedly, cheese was mis-marked. The correct day of the week was checked off, but date was written incorrectly. Foods not used within 24 hours shall be dated the day of preparation and shall be used or discarded in seven days. Cheese was dated correctly during this inspection.
  • R4.4 - The following gaskets are torn: bottom of walk-in freezer, Hobart freezer by fryer (bottom door), 2-door cooler by grill (both doors), reach-in coolers behind front counter (both doors). Equipment shall be in good repair. PIC is working on getting these gaskets replaced.
  • 4.4 - Hobart freezer by fryer is missing a metal panel between the doors and dirt, food debris is falling into the groove. Equipment shall be in good repair. Discussed with PIC.
  • R4.5 - The following equipment is soiled: interior of the fryer doors, cabinet under beverage area, ice build-up around door of walkin freezer, and shelving in walk-in cooler is rusted. Continue efforts to clean these areas thoroughly.
  • R6.4 - Floor tiles are damaged and missing throughout. Coving is damaged and/or missing at back door, around mop sink, and in area of ice machine. Grout between tiles is missing, especially in the grill area. Repair the damaged floor and coving to facilitate cleaning and to prevent pests from harboring in these areas.

  • 2.2- Employee began work, put and single-use gloves, and began food handling activities without washing his hands. Food employees must wash their hands at the beginning of their shift or anytime contamination occurs.
  • 3.0- Open and sealed ham packages floating in a liquid substance. Discard open package and clean and sanitize container and clean outside of sealed ham packages. Fry oil is also extremely black. Food shall be wholesome.
  • 4.4- QAC sanitizer over 500 ppm. Maintain sanitizing solution per manufacturer recommendations or 200 ppm to avoid leaving a toxic residue on food contact surfaces.
  • 3.4/4.4- Front reach-in cooler had an ambient air temperature of 45°F. Milk inside cooler at 47°F. Repair unit in order to maintain
  • 41°F and below. Discard any contaminated foods.
  • R6.4- Floor tiles damaged and missing throughout. Coving is damaged and/or missing throughout. Grout between tiles is also missing. Repair the damaged floor to facilitate cleaning.
  • R6.4- Floor behind and under fryers had food debris build-up. Floor under equipment and in walk-in cooler and freezer is dirty.
  • Thoroughly clean the floor throughout and under and behind equipment.
  • R4.5- The following equipment is soiled: shelf on hot pass thru, interior upright cooler next to grill, exterior of fryers, hood above breading station, interior bottom of upright freezer, shelving in walk-in cooler is dirty and rusty, pooling water under the warming station next to the fryers, and shelf below the grill. Thoroughly clean equipment throughout.
  • 5.1- Three compartment sink and food preparation sink are dripping. Equipment shall be in good repair.
  • R4.4- Gaskets on bottom of upright freezer are torn. Equipment shall be in good repair.
  • R6.1- Large gap under back delivery door. Provide weather stripping or other means to close the gap to prevent pest entry.
  • 6.4- Repair the damaged walk-in cooler floor. Metal piece on floor is separating and needs to lay flat and smooth to facilitate cleaning.
  • 6.4- Clean the soiled walls around breading and fryer areas.
  • Note: This office received two telephone calls regarding the above operation. Reportedly cockroaches were coming out during the day, saw one on menu board, mice droppings in food areas and black mold in walk-in cooler. Baby cockroach reportedly observed in chicken tenders. During this inspection I discussed complaint with the manger. She was aware of the mouse which had been caught.
  • No mice droppings observed during this inspection. Three dead cockroaches were observed in the storage area. A receipt was provided from Akron Pest Control from 10/18/11 setting mice traps and treating for cockroaches. Pest company due back in 30 days.
  • Continue efforts to eliminate cockroach issues and back door needs sealed to prevent further mice problems along with cleaning of the restaurant.

  • 3.4- Sliced cheese holding between 43°F - 47°F. Maintain potentially hazardous foods cold at 41°F and below to prevent bacteria growth. Other foods on ice were holding below 41°F.
  • 3.4- Packages of ham were covered in raw egg. Reportedly eggs were dropped from top shelf earlier in the day. Open packages of ham were discarded. Store raw eggs below ready-to-eat foods to prevent cross contamination.
  • 6.0/6.1- Grout between floor tiles are missing and floor tiles are cracked. Repair the floor to facilitate cleaning.
  • R4.5- Interior of the hamburger freezer is dirty and has frost build-up. Clean and defrost the freezer.
  • 4.5/4.4- Gaskets on upright True cooler are soiled on the top and torn on the bottom freezer part. Clean gaskets and make any necessary repairs.
  • 4.5- Remove the tape around the knife in the holder to prevent possible contamination.
  • 4.4- Shelving in the walk-in cooler is rusty. Resurface or replace shelving.
  • 6.4- Floor under equipment and floor and wall junctures are soiled. Clean the floor more thoroughly.
  • 6.1- Half of back door sweep is missing. Replace this to prevent possible entry.
  • 4.5- Shelf under the conveyor oven has grease build-up. Cart with fry baskets and cart next to biscuit table are dirty. Clean this.
