- 6.1 - Base coving is coming off in back hallway and under hand sink. Floor/wall junctures shall be coved and sealed to prevent water penetrating walls and to prevent pest entry. PIC is working on having maintenance man re-apply coving.
- C/6.1 - Shields missing on the two large bulbs in the back walk in cooler. Bulbs shall be shielded to prevent contamination of food from brokien glass. PIC replaced shields during this inspection.
- 6.4 - Flooring in kitchen area and in the walk-in cooler are soiled. Continue efforts to clean floors in these areas.

- 3.4- Hard boiled eggs on salad bar had an internal temperature between 44°F-48°F. Other foods on the salad bar were at 41°F or below. Potentially hazardous foods shall be held at 41°F and below to prevent bacteria growth. Manager felt container was too full and will put out less in the future.
- C/*7.1- Several bottles of toxic chemicals were stored next to soda syrup and parmesan cheese under front counter. Store toxic chemicals below and away from food and single-use items to prevent possible contamination.
- C/*2.4/6.2- No paper towels in the men's restroom. Provide an adequate supply of paper towels to facilitate hand washing.
- *4.5/4.6- Soda nozzles and interior guard of the ice machine had black residue. Food contact surfaces shall be cleaned to sight and touch. Nozzles were cleaned during this inspection.
- R/C6.4- Coat stored on shelving with straws and bowls. Store employee personal items separate from food and single-use items to prevent possible contamination.
- 6.4- Flies in the kitchen. Provide adequate pest control measures for flies.
- */C2.3- Employees eating from left over pizza stored on side of pizza oven. Provide a separate designated area for employees to eat and store personal items.
- 6.4- Wall in the walk-in cooler is deteriorating. Reportedly owner is aware of problem and is working on a solution.

- 4.5- Following equipment is soiled: dish crates, gaskets on three door pizza cooler, and can rack. Clean these areas.
- 6.4- Clean the floor under shelving in dough area and shelving next to the proofer.
