- 4.8- Waffle maker stored in restroom. Do not store equipment in restroom to prevent possible contamination. Reportedly waffle maker is not being used and will be cleaned and removed from the restroom.
- R3.5- Provide place of business on labels for the gummi raspberry, dark chocolate raisins, milk chocolate malt balls, and chocolate peanuts. Provide complete ingredient labels for pastel blueberry, malt balls, and chocolate cherries with confectioners glaze.

- 6.4- The floor is soiled at floor and wall junctures and under equipment. Thoroughly clean the floor.
- Note: Prior to using the dipper well it needs to be cleaned and sanitized. Ice not being sold at this time.
- Operation may add sundae's to the menu and a commercial blender. Cupcakes may also be added. Once smoothies and milkshakes are added please contact this office 330-723-9523. Preparation of a potentially hazardous food will change the risk level to a level three for the 2013-2014 license season.

- 4.5 Soiled interior surface of reach-in cooler. Clean this. Note: This is not in use at this time.
