  • *2.4/6.2- Second restroom is locked. Reportedly it is opened when it is busy. No soap or paper towels are available in this restroom. Supply this when restroom is open to the public. Reportedly front hand wash sink is not used for hand washing. No soap at this sink either. Provide an adequate supply of soap to facilitate proper hand washing.
  • 4.1- New ice maker added to operation. Appears unit Sunpentown International #IM100 is not commercial grade or approved by
  • NSF or an equivalent agency. Provide verification of this certification or provide equipment that is NSF certified.
  • *4.6/3.0- Bottom of the ice maker where water sits until on to make ice was dirty. Debris floating in water. Remove dirty water and clean and sanitize unit per manufacturer's specifications.
  • R4.5- Walk-in cooler shelving is moldy/soiled. Clean this.
  • R6.2- Replace non functioning light in the walk-in cooler in order to provide 10 foot candles of light.
  • 3.2/4.8- Cocoa and cups stored on floor in the office. Store food and single-use items six inches off the floor to facilitate cleaning and prevent possible contamination.
  • 4.2- Provide a chlorine test strip kit to monitor concentration of sanitizing solution.
  • 3.2- Ice maker is self service. Scoop is stored inside ice maker and sits on top of the ice handle touching ice. Do not store scoop inside unit to prevent possible contamination from the public. Again, appears this unit is not approved in a self serve commercial setting.
  • New Revolution ice tea units added with water line. Be sure a backflow prevention device is installed.

  • This office received a telephone complaint on 9/27/2011. Caller complained no running water in restroom to wash hands, no soap, no papertowels, no toilet paper. Caller also stated bathroom sink, floor, and walls are dirty. Reportedly, flies were also observed.
  • *5.0 Discussed complaint with employee. Water supply is unsafe, therefore, water is no longer available in restroom. Signs are posted pertaining to unsafe water and no running water at handwashing sink. Hand sanitizer was available in restroom today as well as toilet paper. Recommend providing alcohol based moist wipes for handwashing temporarily until this operation is connected to a safe water source.
  • 6.4 Floor, coving, and walls are soiled. Dust accumulation on wall. Clean restroom facilities as often as necessary to keep them clean.
  • No flies observed today.
  • No coffee available for service.
  • Only prepackaged foods available for retail sale.

  • The following are repeat violations from the previous standard inspection report dated 4/20/2011:
  • R/*5.0 The water supply is not safe for human consumption. EPA notices are posted on the restroom door. Provide an approved water source. Water is currently turned off at restroom and employee handwashing sinks. Hand sanitizer located at employee handwashing sink and in restroom. Reportedly, bottled water is used to prepare coffee. Please be advised bottled water must also be used for handwashing and equipment/utensil washing temporarily until a permanent, approved water source is provided. Hand sanitizers may be used in addition to proper handwashing.
  • */R/C3.1 No inspection mark on two packages of Just Mikes Jerky products offered for retail sale. Food must be from an approved source and properly labeled. Person-in-Charge was asked to remove products without the proper inspection mark from the retail sale area today.
  • */R2.4 Person-in-Charge unable to demonstrate knowledge on how to properly clean and sanitize equipment. Person-in-Charge shall be knowledgeable. Educated Person-in-Charge equipment must be washed, rinsed, and sanitized in the three-compartment sink. Educational hand-out provided.
  • 4.2 Quat test kit provided. However, quat sanitizer is not available. Chlorine bleach on premises. Provide a test kit that matches sanitizer to be used. Provide a chlorine test kit to determine proper concentration (50-200 ppm).
  • R6.4/7.1 Observed rodent bait stored uncovered on the floor below the three-compartment sink. Rodent bait shall be contained in a covered, tamper resistant bait station. Take safe steps necessary to eliminate rodents from premises.

  • R/*5.0 Observed filled canisters inside cappuccino machines and power switch is "on". Out of order signs were on the machines upon arrival. The water supply to these machines is not safe for consumption. Provide an approved water source. Reportedly, cappuccino is not served to customers. Advised manager to remove food canisters from machines to eliminate possibility of sale of this product. Manager agreed to remove food from canisters and store empty canisters in the machines. Reportedly, bottled water is used for the preparation of coffee. Please be advised bottled water must also be used for handwashing and equipment/utensil washing temporarily until a permanent, approved water source is provided. Reportedly, operation will be connected to an approved water system within 2-3 weeks.

  • 6.4 Observed water leaking into buckets in the office/dishwashing area. Repair leaks.
  • 6.4 Observed damaged ceiling tiles in office/dishwashing area and restrooms. Replace damaged ceiling tiles.
  • 6.4 Observe miscellaneous items piled in mop sink. Remove these and use mop sink to dispose of mop water.
  • R4.1 Household microwave is not approved for use. Equipment acceptable for use shall be approved by a recognized equipment testing agency (NSF or equivalent). Remove household microwave. Contact Medina County Health Department at (330) 723-9523 prior to installing new equipment for approval. Reportedly, this microwave is for personal use. Observed frozen sandwiches in ice cream freezer which could be heated in microwave. Person-in-Charge agreed to discard frozen sandwiches.
  • 4.8 Observed unwrapped straws available for self-service. Obtain individually wrapped straws or a dispenser to prevent possible contamination.

  • Note: Observed Slim Jim beef jerky products offered beyond recommended use by or sell by date. Check and rotate stock on a regular basis.

  • *5.0 This office received notification today that water sample results were total coliform positive and the presence of E. coli/fecal coliform was detected in the drinking water. You are hereby notified to immediately cease preparation of coffee and/or use bottled water from an approved source for all food preparation, handwashing, and equipment /utensil washing. Drinking water shall be obtained from an approved source. Water from a private water system shall meet water quality standards.
  • Upon arrival, 5 gallons of bottled water were present. Reportedly, bottled water is being used to prepare coffee. Requested out of service signs be placed on cappuccino machines at time of inspection. Person-in-Charge placed signs on machine.
