  • R/*3.4 Observed salami and pepperoni was lacking a date mark. Foods not used in 24 hours shall be dated the day of opening or preparation and shall be used or discarded in seven calendar days.
  • *3.4 Observed the American cheese was holding at 48°F-49°F, and the cut tomato was holding at 53°F on the top side ledge of the reach-in cooler. Foods shall be maintained at 41°F and below to limit the growth of organisms.
  • *7.0 Observed the Raid was stored next to the Twizzlers on the retail shelf. Toxic chemicals shall be stored in a separate designated area from food and equipment to prevent contamination.
  • 3.5 Observed the Turkey and Swiss sub sandwiches, and the ham and Swiss sub sandwiches were lacking ingredients for the submarine roll but contained ingredients for white bread. Correct these packages to contain accurate labels.
  • 4.4 Repair the damaged door seals on the display door on the walk-in cooler storing the wine products, and on the entrance doors on the beer cave, to provide a cleanable surface.
  • R/6.1 Provide a light shield and end caps or a shatter proof bulb on the donut case light fixture.
  • 6.4 Clean the floor drain under the preparation sink, ceiling vent by the walk-in freezer, ceiling tiles and light fixtures surrounding ceiling vents as needed, which are soiled or dusty.
  • 4.4 Repair the surface surrounding the garbage chute which is uncleanable. Provide a cleanable surface in this area.

  • R/*3.4 Observed bologna was dated 7/23/11, and pepperoni was dated 7/23/11. Foods not used in 24 hours shall be dated the day of opening or preparation and shall be used or discarded in seven calendar days.
  • *4.5/4.6 Observed the pizza screens contained food debris accumulation. Food contact surfaces shall be washed, rinsed, and sanitized, and shall clean to sight and touch. Discussed with PIC.
  • *3.2 Observed the bacon was stored above margarine in the retail display case. Different types of raw animal foods shall be stored separate from each other and other raw or ready to eat foods to prevent cross-contamination.
  • *3.2 Observed the Smith's Skim milk was received at 43-45°F, Smith's 2% milk was received at 42°F, and Smith's whole milk was received at 41-42°F. Food shall be received and maintain 41°F and below, to limit the growth of organisms. PIC rejected this milk shipment today.
  • R/4.4 Repair the worn wooden canned food shelves in the rear preparation area, also repair the damaged shelf edge in the cabinet under the cappuccino machine, to provide a cleanable surface.
  • 3.2 Store the bagged ice six inches off the floor to facilitate cleaning and to prevent contamination.
  • 4.4 Repair the damaged door seals on the walk-in freezer door, and the display door on the walk-in cooler storing the wine products, to provide a cleanable surface.

  • Note: Observed the a Chandler beer cave, and Chandler walk-in cooler with custom panels which contain an NSF marking was added to the operation.

  • R/- ceiling vents and/or surrounding areas in the retail area
  • R/- counter under the beverage dispensing machine contains a spill
  • - bottom of the donut case, and interior bottom of the cappuccino machine
  • - floor surface in the rear equipment washing, and in the men's restroom was stained or soiled
  • - floor/wall junctures by the walk-in cooler was stained or soiled
  • - floor surface in the walk-in cooler was stained or soiled
  • - wall and ceiling surfaces throughout as need were stained/soiled
  • - floor under the pop syrup rack was soiled
  • - defrost the interior of the ice cream case which contains ice accumulation. Correct these areas.
  • 4.4/6.4 Repair the following:- front cover on the Federal deli case
  • R/- interior of the waste cabinet in the coffee kiosk
  • - cardboard surface was used to line the access panel under the beverage counter
  • R/- the worn wooden canned food shelves in the rear preparation area
  • - repair the damaged shelf edge under the steam table
  • - scale exterior was cracked, or taped. Provide a cleanable surface in these areas.
  • R/5.1 Clean the urinal in the men's restroom.
  • 6.1 Provide a light shield or shatter proof bulb with end caps on the light fixture in the donut case.
  • 6.2 Repair the nonfunctioning light fixtures throughout as needed, to provide light as specified in OAC 3717.

  • *3.1/6.3 Observed a can of mushrooms was dented on the top seam. Packages of food shall be in good condition to protect the integrity of the contents and to prevent potentially contaminants. Relocate dented foods to a separate designated area.
  • *4.4 Observed QAC sanitizing solution in the spray bottle contained 300-400 ppm. Maintain QAC sanitizing solution at 200 ppm or per manufacturers specifications.
  • 6.4/4.5 Clean the following soiled or dusty areas:
  • R/- ceiling vents and/or surrounding areas in the Market Place and in the retail area were dusty
  • R/- counter behind and under the beverage dispensing machine
  • - observed lime or calcification build-up on the exterior ice machine shield
  • - floor under the walk-in freezer shelves
  • - Ice accumulation under the condenser in the walk-in freezer
  • Correct these areas.
  • 4.4 Repair the following:
  • R/- interior of the waste cabinet in the coffee kiosk
  • - Pepsi machine has water streaming from the fountain heads. Repair man was already contacted.
  • R/- the worn wooden canned food shelves in the rear preparation area, to provide a cleanable surface.
  • R/5.1 Clean the urinal and toilet in the men's restroom.
  • Note: Observed the chocolate and banana Performance bar contained a sell-by or use-by date of 7/2/10 and 6/20/10 respectively.
