  • R/*3.4 Observed spinach, crabmeat, and shrimp thawing at room temperature. Food shall be thawed under running and draining water, as part of the continuous cooking process or under refrigeration to limit the growth of organisms.
  • *3.4 Observed 5 containers of cheese queso was cooling at room temperature and contained a temperature of 76°-78°F after more than 2 hours of cooling. Foods shall be rapidly cooled from 135°F-70°F within two hours and from 135°-41°F within a total of six hours, to limit the growth of organisms.
  • *6.2 Provide and adequate supply of paper towels to promote proper hand washing in the employee restroom.
  • R/6.4 Repair the damaged wall surface in the employee restroom, and in the waitress area to provide a cleanable surface.
  • R/6.2 Repair the non-functioning light fixtures throughout, to provide lighting requirements as specified in OAC 3717.
  • R/4.4/6.4 Observed the ice machine is leaking, also clean the floor under this unit. Clean and repair this area.

  • Reportedly repairs are scheduled in the near future to repair the lighting, damaged wall surfaces, leaking ice machine, damaged floor tiles, and base cove.
  • Operation added a Mc Call reach-in unit. This unit replace the unit which was pooling water.

  • C/PROTECTION FROM CONTAMINATION: Observed cut tomatoes were stored on top of raw chicken under a bin of raw meats. Store raw animal foods separate from other types of raw animal foods and ready-to-eat foods to prevent cross contamination. This food item was discarded today.
  • PROTECTION FROM CONTAMIANTION: Observed the cook's lunch plate in the kitchen and a beverage with a straw. Employees shall eat and consume open drinks in an separate area. Employees are permitted to consume beverages in a closed container throughout the kitchen.
  • THAWING: Observed the spinach was thawing in pooling water. Thaw the spinach under refrigeration, in running draining water, or as part of the cooking process.
  • *4.5/4.6 Wash, rinse, and sanitize the soiled slotted spoon on the equipment rack. Food contact surfaces shall be clean to sight and touch.
  • *4.0 Observed the tortillas were stored in the to-go carry out bags. Store foods in food grade bags or containers to prevent imparting of odor, or tastes.
  • R/4.4/4.5 The bottom of the McCall 2 door reach-in cooler has pooling water. Clean the water and have the unit serviced as needed. Equipment shall be in good repair.
  • 4.1 Provide thermometer in the reach-in unit closer to the office. Monitor the ambient air temperature.
  • 6.4 Repair the damaged wall surface in the employee restroom, and in the waitress area to provide a cleanable surface.
  • 6.2 Repair the non-functioning light fixtures throughout, including the employee restroom, cook's area, and above the mixer to provide lighting requirements as specified in OAC 3717.
  • 4.4/6.4 Observed the ice machine is leaking, also clean the floor under this unit. Clean and repair this area.
  • 6.4 Clean the dusty ceiling vent and ceiling tiles surrounding area.
  • 3.2 Store the onions six inches off the floor to facilitate cleaning and to prevent contamination.
  • 4.5 Clean the exterior of the shredding attachment, and the hood unit above the dish machine which is soiled.
  • 6.4 Repair the damaged floor tiles surrounding the floor drains, by the mop sink, and under the oven, by the McCall refrigerator, to provide a cleanable surface.
  • 4.4 Repair both damaged door seals on the True reach-in unit, and the damaged lid on the Texican Tortilla chip unit, to provide a cleanable surface.
  • 3.2 Label the clear containers with a white lids with the name of the food to avoid confusion and misuse.
  • 6.4 Provide the lacking base cove under the hood unit by the office, to provide a cleanable surface at the floor/wall juncture.
  • 4.0 Remove the garbage bag lining the side of the Mc Call unit. Provide a cleanable surface.
  • 3.2 Provide a lid for the rice container to prevent contamination.
  • Note: The employee restroom contains a sewer gas type odor. Repair this area.
1/9/2012Critical Control Point 1Accurate long stem thermometers were available.
Reportedly, proper reheating on the stovetop to 165°F within 2 hours.
PROTECTION FROM WASHING HANDS: Observed the no paper towels were available for use in the employee restroom.
Provide an adequate supply of paper towels to promote proper hand washing. PIC was asked to provide this today.
Reportedly, refried beans properly cool in shallow pans in the walk-in cooler.
HOLDING: Refer to the temperature log below. Foods marked with an asterisk were holding at improper temperatures. Foods and refrigeration units shall be maintained at 41°F and below to limit the growth of organisms.
The PIC contacted a refrigeration man for repairs today. PIC discarded food items holding above 45°F at the recommendation of this inspector. Additionally Shredded chicken 75°F, and Pork 51°F were holding at improper temperatures.
  • 6.1/6.4 The back door is open. Keep the screen door closed to prevent pest entry. The screen door is torn. Repair the screen so the outer opening is protected.
  • 4.4/4.5 The bottom of the McCall 2 door reach-in cooler has pooling water. Clean the water and have the unit serviced as needed.
  • Equipment shall be in good repair.
  • R/6.4 The mop is leaning on the soda boxes. Store maintenance tools in a separate designated area.
  • 4.2 Provide thermometer in the salsa refrigerator and in the Frigidaire refrigerator in the bar. Monitor the ambient air temperature.
  • 6.4 The floor drains are soiled. Clean these.
  • 5.4 Maintain dumpster lids closed between use. Clean the dumpster area. Discard cardboard.
  • Note: New Frigidaire (CSA) #LFPH33MYLM refrigerator was added to the operation.

  • 4.5 Continue to clean the area between the fryer and grill.
  • 5.4 Clean the dumpster area. Discard cardboard and unused equipment. Collection of the items is scheduled.
  • Note:
  • Keep the screen door closed to prevent pest entry.
  • The handsink is scheduled to be prepared today. Use the restroom handsink in the mean time.
