03/04/2015Re-InspectionConversion kit for outdoor grill has been installed and grill is now hooked up to natural gas. Hood and wall behind fryer has been cleaned. Flooring in beer walk in cooler has been cleaned. Owner was ordering a door for the men's bathroom during the inspection. Owner placed mouse traps in the back storage room and has caught several mice. Owner to look into how the mice are entering the facility and seal up any holes that are found. Owner to seal up the flooring behind to bar.

01/30/2015InspectionReceived application, wc/db, check #2660 in the amount of $150.00 and $50.00 cash for a total of $200.00 for annual health permit. Inspector to change the facility from low risk to medium risk because they started using the kitchen and cooking more foods. Exterminator to be hired to deal with the rodent problem. Owner must provide a copy of a receipt or contract with the Extermination company. Facility is currently using an outside propane grill to cook, spare propane tank onsite as well. Owner ordered a conversion kit so that the grill can be used with natural gas that is already available in the kitchen. Owner to contact OCHD when conversion for grill has been made as well as when the flooring behind bar has been addressed and after exterminators visit. Reinspection in 3 weeks.
