Assorted foods were held at 50-54 degrees F rather than 41 degrees F or below as required. A list of all out of temperature potentially hazardous foods was provided and is in the district file. Foods were discarded.

The bay utensil sink was not properly set up. the wash and sanitizer solutions were in bays right next to each other. The center bay must must be set up for the clear water rinse. wash-rinse-sanitize.

Moist wiping cloths observed lying on prep table and not stored in sanitizing solution. Use sanitizer pals provided.

A employee used a black hose non NSF/ANSI hose attached to the mop sink to close clean the 3 bay NSF utensil sink and then to fill the bays with water. There is nothing wrong with the faucet of the 3 bay utensil sink. All the bays must be cleaned and sanitized before use.

Visible physical evidence of rodent activity observed in the utility/office room. Numerous fresh mouse droppings were observed on the floor along the wall and in corners. Fresh mouse droppings were also observed along walls and in corners in the rear prep areas.
