Off duty Employee observed walking in & out of kitchen without donning a cap or hairnet. Air gap on both ice machines are not present. Hot water at hand sink is less than 100 degrees. Any plumbing repairs must be performed by a licensed plumber and must be repaired according to law. All materials used for plumbing repairs must be of materials approved according to law. Hand sink has no hot water. Repair or replace radiant heat window units. Eliminate the presence of flies in facility by using any approved means. Install additional air curtains at all door entrances and open service counter openings. contact a licensed Pest Control Operator for assistance, and have receipts from Pest control to verify this. Observed that the three compartment sink was not properly set up; during food service and warewashing. Resulting in a 000.00ppm FAC reading in the sanitizing compartment. 2 of 4 sanitizing pails had a 0.0 ppm FAC reading. 6" crack in the bin of the ice machine. Food contact equipment must be in good repair. The Bag-In-Box connector and tubing are covered with a syrup and dirt residue. Mold growth on the inside of both Ice machines. Clean & Sanitize all food contact surfaces. Ice scoops must be stored in an NSF approved holder. Ice pails must be stored upside down.	06/23/2010	80
