- Observed hard boiled eggs in the salad cooler at 48 F. The manager voluntarily threw the eggs away. He stated he will contact a repairman.
- Observed the following out of date: bag of chicken in a reach in cooler 2/1/16, bag of sliced cappicola 1/28/15, bag of sliced ham 2/9/16. All were voluntarily discarded by the manager.
- Observed 2 employees handling food without hair restraints. Employees should wear effective hair restraints such as a hat or a hair net.
- Observed a bag being used at the end of the dish machine. Per the owner a wall is being made to re place the bag.
- Observed the standing reach in freezer with food build up. Handles of reach in coolers throughout are sticky and have food debris build up. Clean and maintain routine cleaning.
- Observed debris build up on the floor by the standing reach in freezer. Clean and maintain routine cleaning.
- Observed cool down documentation being taken at onset of cool down and nothing further. Cool down documentation should be recorded at the onset of cool down then in 2 hours and again in 4 hours. I will attach a cool down handout to the e-mail containing this report.

- Found duck tape used to hold wiring in place in kitchen by grill & on end of overhead air duct. Also, need to cap end of air duct in kitchen.
- Found lent/grease scum/soil on some electr boxes, pipes, wires, etc in kitchen.
- Found wet mop stored in mop pail.

- Observed missing equipment shielding below cold holding/ pizza prep table exposing motor unit. Please ensure all non-food contact surfaces are easily cleanable and in good condition.
- Observed rust build-up on metal racks in dish-room where cleaned items are placed to dry. Please ensure racks are cleaned to prevent the chance of physical contamination.
- Observed metal racks above pizza topping/ cold holding station have dust build-up. Please ensure shelving is clean to prevent contaminants from falling into the food.
- Observed soda spill in basement below soda lines. Please ensure floors are clean to prevent slipping hazard or pest issues.
