  • Observed the employee restroom garbage bin does not have a lid. Provide lid.
  • Observed the hand sink by the soda machines with no hand soap. The manager provided hand soap during the inspection.
  • Observed stale soda spills in the walk in cooler and under the soda dispense boxes in the back room. Clean and maintain routine cleaning.
  • Observed the mop stored in dirty mop water.

  • In the walk in cooler there was a container full of opened hot dogs, brats, and tornadoes that weren't labeled with the date they were opened. They were date labeled. Explained date labeling to employees. They are only good for 7 days after opening (the day they're opened counts as a day). You must train all employees about date labeling.
  • Observed an employee using the hand sink by the soda fountain as a dump sink for coffee. Discussed with manager that that sink is only for hand washing since they have food in that area. Employees must use the utility sink in the back to dump drinks.
  • There is cardboard used under a bag of water softener salt under the hand sink in the back area. The cabinets under the soda fountain appear to have water damage. There is also unsealed wood in the storage area. I pointed this out to the employee.
  • There is buildup in the ice machine. The cabinets under the soda fountain are very sticky and have areas were drinks have been spilled. There is also rodent feces in at least one of the cabinets. Do not store any food contact surfaces (cups) in the cabinets until they're cleaned and free of rodent feces.
  • Found no paper towels at handsink by the soda fountain.
  • Dumpster lid open outside.
  • There are mouse feces in the cabinet underneath the hand sink by soda fountain. The facility is treated by a licensed pest company. The manager was on vacation and other employees were unable to find the invoices. Per employee there are mouse traps set up throughout the facility. Discussed contacting the pest company about the mouse feces.
  • The floors in the storage area and the water softener area are dirty and sticky. There are also areas where the floor is dirty in the main shopping area. The water softener area is dirty and has salt on the floor.
  • The mop was stored in the dirty mop water.

No violation noted during this evaluation. 08/25/2015Follow-up100
  • In the walk in cooler there was a container full of opened hot dogs, brats, and tornadoes that weren't labeled with the date they were opened. They were voluntarily disposed of due to not knowing when they were opened. Explained date labeling to employees. They are only good for 7 days after opening (the day they're opened counts as a day). You must train all employees about date labeling.
  • There were sandwiches hot holding at 110f. There was no time on them. The employee did not know when they were put in there. They were voluntarily disposed of. The unit was kept at 110f which is too low to hot hold which should be 135f or above. You must do time instead of temperature making sure you label the product with the time you put them in there and are used within 4 hours. You should have a procedure for this and educate all employees.
  • I wasn't able to locate a metal stemmed thermometer. Per employee the manager takes temperatures. All employees should know where the thermometer is.
  • There was a bag of soda going to soda lines that was on the floor. The employee moved it to the shelf. It also appeared to be leaking.
  • The cabinets under the soda fountain appear to have water damage. There is also unsealed wood in the storage area. I pointed this out to the employee.
  • There were rags by the handsink close to the soda fountain. They must be stored in sanitizer in between uses.
  • The cabinets under the soda dispenser are very dirty and are black. There appears to have been a leak at some point. There is also mouse feces and crickets. Do not store anything in them until it is cleaned. Also the handles to open the cabinets are very sticky.
  • Behind the ice machine there is standing water. I pointed this out to the employee as there may be a leak.
  • In the storage area the hand sink was blocked by several cases of soda and had an onion slicer in the sink. The soda and slicer were moved. Explained that all hand sinks must be accessible.
  • The dumpster lid was open.
  • In the back storage area there are several dead insects. In the cabinets under the soda fountain there is mouse feces and a couple insects (1 live). This needs to be cleaned so that you can monitor if there is an ongoing issue. The employee believes that the facility gets treated by a licensed pest company. I need to see invoices to verify this as there is a pest issue.
  • The floors in the storage area and the water softener area are dirty and sticky.
  • There is a missing ceiling tile with exposed insulation above the soda lines in the storage area. I pointed this out to the employee.
  • Above the mop sink there were several cleaners including bathroom cleaner stored next to cups some of which were opened. Also on the 3-compartment sink there were hot dogs stored below detergent. The employee thinks the hot dog buns are going to be disposed of. The cups were moved away. Make sure all employees know about chemical storage.
