- Observed lid of pizza prep cooler broken off. Lid has been broken since last inspection in December and per manager, it is scheduled to be repaired when cleaning and repair crew visits the facility. Manager temporarily put lid back on the cooler. Also observed broken gaskets of walk in cooler. Repair or replace gaskets.
- Observed debris buildup on metal food storage shelves. Ensure routine cleaning of all non-food contact surfaces.
- Observed debris buildup on floors around equipment and along walls; specifically near dish machine and in corner of store. Routinely clean floors.
- Observed damaged floor tiles throughout the kitchen, primarily near dish machine and along walls. Ensure all floor tiles are smooth, durable, nonabsorbent, easily cleanable, and in good repair.
- Observed holes in ceiling tiles in kitchen. Ensure all ceiling tiles are in good repair.
- Observed heavy debris buildup on ceiling tiles above utility sink area, pizza prep area, and above pizza oven. Clean ceiling tiles. Also observed debris buildup on walls around dish machine and 3-compartment sink. Clean walls.
- Observed heavy debris buildup on exhaust vents on ceiling and in vents of oven. Clean all vents.
- Observed several pieces of equipment and miscellaneous items being stored in corner of kitchen, near utility area. Remove all excess equipment unnecessary for the operation of the store to eliminate pest harborage areas.

- Found macaroni salad, potato salad, jello-whipped cream dessert, diced cantelope, salad dressings, and puddings at 46-50°F on the salad bar. These foods were voluntarily discarded. Maintain at 41°F or below. The unit was turned to a cooler temperature during the inspection and was at 36°F at the end of the inspection.
- Observed employee grating cheese for the pizzas and salad bar with no gloves. Wear gloves when handling ready-to-eat foods. Employee was immediately retrained on site.
- Paint or replace the very rusty shelves in the paper product storage shed behind the building.
- Found a dirty towel sitting on top of the vent by the pizza make table in the kitchen. Store it in sanitizer between uses.
- The top of the pizza make table, and shelves throughout the kitchen had a build up of debris. Clean and sanitize these areas.
- The fan in the walk in cooler and outside of the door to this cooler had a build up of debris. Clean and sanitize them.
- Replace the missing drain cover under the Point of Sale Station in the kitchen. Replace missing floor tiles in the kitchen under the dish machine.
- Found build-up of debris on floors. Clean the floors.
- Clean the dusty and stained ceiling tiles in the kitchen.
- Hang up mops and brooms on the floor throughout the facility.

- Observed employees not wearing effective hair restraints. There was only 1 employee in the kitchen wearing a hat. Per manager hats have been ordered and the corporate policy states that employees are to wear a hat when doing food prep.
- Paint chipping along edge of large stand doughmaker. The side of the door to the walk in cooler has the seal coming off and the coating coming off. I pointed this out to the manager.
- There was some buildup in the handles of the reach in cooler at the bottom of the pizza prep table. Also there was buildup on the racks holding cans and other foods.
- Found build-up of debris on floors including leaves in several places. Clean the floors.
- Food build-up on walls especially behind the dishmachine. Also there is dust accumulation on the ceiling and on lights.
- Found unlabeled spray bottle of chemicals by the 3-compartment sink. It was labeled by the manager.
- In the mop sink area there were some pizza boxes stored under a box of floor cleaner and there was bleach in the mop sink. Pizza boxes are a food contact surface and should be stored in a place where they can't be contaminated. They were moved.

- Observed employee improperly washing their hands. Pizza maker washed hands and then touched handle of sink with bare-hand. Suggest training on proper hand-washing technique with all employees i.e. use paper towel to turn off water. Provide information on training via email to inform what was discussed and who attended by 08-08-2014.
- Paint chipping along edge of large stand doughmaker. Suggest contacting manufacturer for maintenance of doughmaker.
- Lid needed for women's bathroom garbage.
- Found what looked like mold build-up and water damage on walls behind dish machines.
