- Container of taco meat was not labled with 7 day date. Also, frozen sloppy joe meat was not labeled with correct date. Both products were labled with correct dates.
- Found open cup of soda in kitchen with no cover. Mgr discarded cup of soda.
- Found grease/scum buildup on bottom trey of pizza oven.
- Found frost/ice buildup inside chest freezer.
- The cement blocks/pad for dumpster are broken & not cleanable.
- Some paint on kitchen floor is worn/missing/dirty & hard to clean.

- Observed a lot of exposed wood in the dry storage area. Seal all exposed wood including on shelves and the around the door frame.
- Observed food debris on the pizza oven rack. Wash, rinse, and sanitize rack on a routine basis throughout the day.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the outside of the pizza oven and the racks in the walk in cooler.
- Observed dust/dirt build up on the floors especially under equipment and in corners. Clean the floors.

- There was an opened gallon jug of milk that had been opened with no label of the day it was opened. The sell by date on the jug said 01-06-15. Manager disposed of the milk at time of inspection. She said they only use it on Wednesdays .
- All food contact surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the ice machine drip tray and soda dispenser nozzles.
- All food contact surfaces used for cooking need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the pizza oven rack.
- All non-food contact surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the panel behind the grill, the bottom of the reach in cooler, and the door seals on the reach in coolers/freezers.
- Observed 1 round pan and one rectangular pan stored on the floor near the hand sink. Equipment needs to be stored at least 6
- Build-up on floors especially under equipment. Clean the floors.
- Build up on the wall by the sinks. Dark build up on the ceiling in the walk in liquor cooler. Clean the walls and ceiling.
- Observed mop on the floor near the utility sink. Mop needs to be stored in a manner that allows it to air dry.
