Upstairs Kitchen: Observed unlabeled container of beef pasta in cooler. Per manager, she did not know when pasta was made. Manager voluntarily discarded beef pasta. Also, observed 2 trays of cooked spaghetti with a date label of 12-3-2015, 12 days past the day of preparation. Manager voluntarily discarded both trays. Ensure employees are correctly date labeling food with date of preparation as Day 1 and storing foods for no longer than 7 days.
Upstairs Kitchen: Observed unsealed wood cabinet being used for food storage. Seal all wood shelves or remove wood cabinet from facility.
Downstairs and Upstairs Kitchen: Observed food debris in insides of microwaves. Ensure routine cleaning of all food contact surfaces.
Downstairs Kitchen: Observed leaking 3 compartment sink. Contact plumber and repair faucet. Upstairs Kitchen: Observed 3 compartment sink faucet that does not close fully, causing it to leak. Contact plumber and repair faucet.
Downstairs Kitchen: Observed holes in corner of walls around pipes. Ensure all pipes are tight fitting to eliminate gaps and areas pests can travel through.
Downstairs Kitchen: Observed heavy dust buildup on vents above stove. Routinely clean vents. Upstairs Kitchen: Observed dust buildup on vents above stove. Routinely clean vents.
Upstairs Kitchen: Observed ceiling light fixtures without protective shielding. Ensure all lights have proper shielding to protect food and surfaces in case light bulb cracks or shatters.
Upstairs Kitchen: Observed unlabeled chemical in spray bottle. Ensure all cleaning spray bottles are correctly labeled. Manager labeled bottle on site.
There is no certified IDPH Food Manager at the facility. Facility has 90 days to comply, contact IDPH at DPH.FSSMC@illinois.gov. Must have certified manager no later than 3/14/2016.
