- Observed a hole in the top of the table adjacent to the toast table and rusting shelves in back storage area. All food contact surfaces must be smooth, easily cleanable, and maintained in good repair. Repair or replace.
- Observed damaged and rusting shelf under dishwasher table. All non-food contact surfaces must be smooth, easily cleanable, and maintained in good repair. Repair or replace.
- Observed duct tape on the pipe entering the grease trap. Duct tape is not approved for use in food service as it is not smooth and easily cleanable. Repair or replace.
- Observed wet table cloth on the bottom of the prep table. All wet towels must be placed in sanitizing solution or laundered after use. Manager disposed of towel during inspection.
- The sanitizer bucket in the kitchen registered less than 100 ppm quaternary ammonia. Discussed with managers and employees that the sanitizer in the bucket must be at least twice the strength of what is used in the three compartment sink. Manager stated that employees should use 1 1/2 to 2 tablets to ensure this. Employee remade the bucket during inspection.
- Observed gap along bottom of the door entering the personal garage. The personal garage door also had a gap. Repair either the garage door or the entrance door by 2/18/16 to deny pest entrance into the facility.
- Observed damaged floor of the walk-in cooler and damaged floor in the back of facility. Repair or replace to allow for proper cleaning.
- Observed build up of debris underneath three compartment sink. Clean as routinely as necessary.
- Observed damage bottom left corner of door entering dry storage. Repair or replace.
- Observed bucket on three compartment sink to measure greater than 400 ppm quaternary ammonia. Per employee, he used two tablets and the manager stated he needs to start using one. Quaternary ammonia in a sanitizing bucket must be at least twice the strength of the three compartment sink but less than 400 ppm. Employee dumped half of the bucket and filled it with water, the concentration was around 250 ppm.
- Observed greaser chemical stored on shelf with food contact surfaces. All chemicals must be stored physically separate from food and food contact surfaces. Manager removed chemical during inspection.
- To be in compliance with the code, facility must create a list of all employees which includes the hire date (new employees only) and their date they received certification.

- Observed the following items without proper 7 day date labeling: hashbrown patties, slices of pepper jack cheese, sliced ham, sliced roast beef, and sliced cheese. All commercially processed foods removed from original packaging must be labeled with 7 day date label. Kitchen manager stated he sliced all the meat yesterday and did not know that it needed to be date labeled. Employee labeled all foods with proper 7 day date label.
- Observed waitress handling toast out of toaster oven with bare hands. Ready to eat foods must not be handled with bare hands. Manager provided tongs at the toast station to ensure there is no bare hand contact with toast.
- Observed towels on table next to dish washing machine. Food contact surfaces must be non-absorbent and easily cleanable. Remove all towels from food contact surfaces.
- Observed peeling paint and rust form on bottom shelves of dish washing tables that hold clean food contact surfaces. All food contact surfaces must be smooth, free of abrasions, and in good repair. Repair or replace.
- Observed extensive damage to cabinet beneath waitress area sink. Repair.
- Observed exposed untreated/unpainted wood in the mop/utility area and on the floor in the back storage area. Untreated wood is not approved for food service as it is absorbent. Paint/seal/replace as necessary.
- Facility had test strips for quaternary ammonia but are using bleach (chlorine) for their sanitizer. Must be provide proper test kit to ensure proper concentration is being met.
- Chlorine concentration in sanitizer bucket used for dining surfaces and cooking surfaces was less than 50 ppm. Sanitizer must maintained between 100-200 ppm for spray bottles/sanitizer buckets.
- Observed large amount of debris/dust accumulating on top of soda dispensing motors. Clean as routinely as necessary.
- Observed hand sink in dish washing area without running water. All plumbing shall be maintained and in working condition. Repair.
- Observed gap forming along bottom of garage door. This gap is large enough for pests to enter into the facility. Repair gap along bottom of garage door by 12/20/2015.
- Observed the following floors to have a build up of debris: floors in back storage area, floors in dish washing area. Clean routinely as necessary.
- Observed broken floor tiles in kitchen, back storage area in front of walk in freezer, and in front of the door entering the dry storage area. Repair to allow for proper cleaning.
- Observed the following walls/doors in need of repair: wall by back storage area door has holes from door knob and at floor to wall juncture. Repair.
- Observed clutter accumulating in back garage area. Remove all items that are not necessary to the facility to ensure pests are not provided harborage area.

- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to racks, seals, handles, and the bottoms of the reach in coolers.
- The hot water to the hand sink near the 3-compartment sink is not working. Hot water is flowing to all other fixtures in the facility. Contact a licensed plumber to repair the sink.
- The backdoor has a gap along the bottom that daylight is visible under. Keep back door closed until weather stripping is applied or back door is replaced. Repair door by 07-20-2015.
- Observed cracked and broken floor tiles throughout back storage area. Repair doors.
- Observed build up on floors especially under equipment and in corners. Clean the floors.
- Observed gaps in the ceiling around piping. Observed holes in walls and pealing ceiling paint in the back area. Seal all gaps and holes. Repaint ceiling.
- No light shields in the dining area. Replace light shields.

- Observed 2 employees prepping/cooking food without wearing hair restraints. Hair restraints (hats, hair nets, bandanas, skull caps) must be worn in food prep area.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the soda line racks, cold storage racks, handles, and door seal gaskets.
- Observed rubber scrapers and whisks improperly stored. Always store cooking utensils with the handle presented to the employee.
- Observed cracked and broken tiles in the dry storage area. Repair or replace the tiles.
- Observed a puddle of water coming from the utility area and build up on the floors especially under equipment. Clean the floors.
- Observed holes in the walls in the dry storage area near the door and exposed wood on the walls in the utility sink area. Repair the walls.
- Observed an unlabeled spray bottle of cleaner. The spray nozzle was broken so an employee disposed of the bottle. Always clearly label bottles of cleaner with their common name.
