- Observed sliced meat in glass door refrigerator in kitchen that did not have date label. All foods removed from original packaging must be dated with 7 day date label. Per owner, she did not know what date they came out of the freezer. Owner discarded the meat during inspection.
- Observed cheese in glass door refrigerator without date labeling. The facility receives cheese in large quantities and individually portions them, then freezes them. When the individual portions are removed from the freezer, they must be labeled with proper 7 day date label. Per owner, Sammi, the cheese was thawed on Monday. She added mondays date to package during inspection.
- Observed employee wash hands and use bare hands to turn off faucet. To properly wash hands, employees must use single service towel to turn off faucet to prevent contamination of clean hands.
- Observed employee drinking from cup in bar without lid or straw. Employees are allowed to drink in food areas but must have lid or straw to mitigate hand to mouth contact and prevent spills.
- Observed employee leave bar area to prep food without hair restraint. All employees handling food must wear an effective hair restraint (hat, hairnet, bandana).
- Observed towels/paper towels in bucket holding tongs by fryer, under utensils stored in tool box, and under cups/pitchers in refrigerators in bar area. Food contact surfaces must be non-absorbent to prevent contamination. Remove towels from food contact surfaces.
- Observed exposed wood on floor in walk-in cooler. Exposed wood is absorbent and all non-food contact surfaces must be non-absorbent. Paint, seal, or replace.
- The floor in the back is a combination of tile and cement and is not easily cleanable. Remove tile or retile the rest of the floor to make it easily cleanable. The floor in walk-in cooler has paint peeling. Repaint floor to allow for proper cleaning.
- Observed build up of debris on floor underneath and behind soda lines and underneath the soda equipment. Clean as routinely as necessary.
- Observed peeling paint on ceiling in back storage area and on door trim near arcade machines. Repaint, reseal, or replace to allow for proper cleaning.
- Observed aspirin stored with alcohol and food contact surfaces in back storage area. Personal medication must be stored separate and away from food and food contact surfaces.
- Observed unlabeled chemicals hanging on service sink in back area. All chemicals removed from bulk containers to individual buckets or spray bottles must be labeled to prevent misuse.

- Observed bartender cooking food without wearing a hair restraint. Her hair was only restrained with a hair tie. Always wear a hair restraint (hat, hair net, bandana) when working with food.
- Observed exposed wood in bar area and in cooler. Seal all exposed wood. The door seal on the reach in cooler in the back area is broken. Replace seal.
- Sanitizer bucket behind the bar was low ~10 ppm. Sanitizer also appeared to be dirty. Replace sanitizer with clean water and dilute to 100-200 ppm for chlorine sanitizer.
- Observed build up in the soda dispenser nozzle. Wash, rinse, and sanitize on a daily basis.
- Observed build up in the microwave. Wash, rinse, and sanitize the microwave on a routine basis.
- Observed build up on the soda nozzle holder. Wash, rinse, and sanitize on a routine basis.
- Observed a Y-splitter attached to the faucet of the utility sink. These are not allowed as they do not prevent backsiphonage. Remove the Y-splitter.
- The floor in the back is a combination of tile and cement and is not easily cleanable. Remove tile or retile the rest of the floor to make it easily cleanable.
- Observed build up on floors especially under equipment and in corners. Clean the floors.
- Observed chipping and peeling paint on the ceiling in the back area. Repaint the ceiling.
