- Observed open package of hot dogs and opened tornadoes with no date label. Label potentially hazardous foods with 7-day date label of when it needs to be thrown away. Manager threw hot dogs away.
- Observed exposed wood on shelves throughout back area. Repaint/seal all exposed wood. The top of the reach in freezer in the back area is cracked. Replace lid. There is a piece of cardboard under the Rockstar cooler. Cardboard is not easily cleanable. Remove cardboard.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the hand sink, seals, handles, and the soda grates.
- The faucet on the 3-compartment sink sprays out where the faucets joins the basis. Contact a licensed plumber to repair the sink.
- The garbage can in the unisex bathroom has no lid. Provide lidded garbage cans.
- There were no paper towels in the unisex bathroom dispenser.
- Dumpster lid was open at time of inspection. Keep dumpster closed at all times.
- There is carpet under the hand sink in the back area. Carpet is not non-absorbent. Remove carpet.
- Observed build up on floors in back area especially under equipment and in corners. Clean the floors.
- Observed gaps in the walls throughout back area. Seal all gaps. The wall-floor juncture near the walk in cooler is crumbling away. Repair wall.
- There is a gap between the 3-compartment sink and the wall. Seal sink to the wall.
- Found build-up on walls throughout back area and in walk in. Clean the walls.
- Most of the lights are not shielded. Facility is in the process of replacing lights. Do not remove light shields until new lights are ready to be put in.
- Observed mop stored wet in bottom of bucket. Store mop so that it may air dry. Mop may be stored in grated portion of mop bucket.

- Tornadoes in reach in cooler in dish area uncovered. Monitor food in storage to ensure it is always completely covered.
- Broken cabinet door near coffee station. Wood showing through in cabinets in back area near walk in cooler and near soda dispensers. Racks in sandwich cooler have rust on them. Racks in the walk in cooler have rust on them. Soda line racks have paint chipping. Repair or replace these items.
- All food contact surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the cream/sugar dispenser nozzles.
- All food contact surfaces used for cooking need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the inside of the microwave.
- All non-food contacts surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the coffee dispenser grates, plastic racks in the walk in coolers, soda line racks, the top and sides of the reach in freezer near the 3-compartment sink, cooler door seal gaskets, the top of the ice cream case near the point of sale station, and behind the soda and coffee dispensers in the lobby.
- The handsink in the back is blocked by a sign. Remove the sign and keep the path to the handsink clear.
- Dumpster was full with boxes and the lid was open. Suggest breaking boxes down before putting them in the dumpster to allow the to close.
- Found a large hole in the floor/wall juncture near the walk in cooler. No light is shining through so there does not appear to be an opening for pests. Repair the hole. Cracked and chipped floor tiles throughout the back area.
- Dust build up on the floors especially under equipment. Clean the floors.
- Found 2 holes in the ceilings beneath what appear to be air vents. Cover these holes with a proper screen. Ceiling tiles out of the grid in the back area. Replace tiles.
- Build up and food splash on walls throughout back area and walk in cooler. Clean the walls.
- Dust build up on air vents throughout the facility. Clean the air vents.
- Cracked light shield in the dish area. No light shield in storage area near point of sale station. Repair the light shield in the back and provide a light shield to the other light.
- Box of candy bars stored on the same shelf as a bottle of cleaner. Move box of candy bars out of dish area to prevent contamination. Do not store any food products on the same shelf or below cleaning products. Monitor storing practices to prevent cleaners from contaminating food products. Employee moved candy bars.
