- Observed soda line nozzles being sanitized inside the den's hand sink. Hand washing sinks must be accessible at all times and only used for hand washing. Manager removed the nozzles and told employee in charge of area.
- Observed the following food contact surfaces in need of repair: metal shelving holding foam trays in dry storage room is rusting, peeling seal on wood shelf holding soda in dry storage room, wood platforms holding items 6

- Observed the following non-food contact surfaces in need of repair: paint peeling on edge of Tyler walk-in refrigerator door, duct tape on sink by Groen equipment, duct tape on chair used to hold ice scoop, peeling paint on shelving below chips in service line, and duct tape on the metal cart in the den. Duct tape is not an approved material for food service. Repair or replace these damaged items with approved materials to allow for proper cleaning.
- Observed moist wiping cloths stored in the sink bay to the left of the three compartment sink. All moist wiping cloths used to wipe food contact surfaces must be stored in sanitizer or laundered after use.
- Observed a leak in the faucet near the door exiting to the cafeteria in the dish room and the hand washing sink faucet in the den. All plumbing must be maintained in good repair.
- Observed garbage dumpster lid open outside of facility. Dumpsters must remain closed to prevent entrance of pests. Dumpster was closed during inspection.
- Observed the following areas of the floor in need of repair: crack in floor entering dry storage, damaged floor tile near kelvinator cooler, crack in floor tile by door to staff service area, broken tile by swinging exit door, damaged seal/paint on wood riser in service area. Repair or replace to allow for proper cleaning.
- Observed ecolab chemicals stored on shelf next to single service food trays and on the containers next to single service coffee lids. Chemicals must be stored physically separate from food contact surfaces. Manager removed chemicals during inspection.

- Observed wood shelves in the dry storage room and wood riser in the cafeteria line that are not sealed. Seal so these areas are easily cleanable. In the pizza service room there is coving that is missing along the cash register line and a hole in the bottom of the wall. Repair.
- Observed the seal to the old walk in cooler with black debris build up. Wash, rinse, and sanitize. The outside of the cooler in the dish machine room is sticky. Non food contact surfaces such as the doors and handles of coolers should be regularly cleaned.

- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to hand sink seals, faucets, and the outside of the microwave.
- The seal on the hand sink is severely chipped away. Reseal sink.

- The seal on the tiles near the prep sink by the ovens in the back of the kitchen are cracking. The seal behind the handsink in the dish area is cracking. Reapply sealant.

- Observed build-up on the handles of multiple cabinets throughout the kitchen, on the racks in the cafeteria dish area, and the door seal gaskets of the walk in coolers and freezers. Wash, rinse, and sanitize these surfaces.
- Observed metal dishes in dry storage stored with food contact surface present. Store upside down.
- The ceiling in the walk in cooler near the cafeteria has a gap between the wall and the ceiling in one of the corners. The sealant on the walls and ceilings in all the walk ins is beginning to chip. Please repair.
- The walls the run along all the walk in coolers and freezers have a brown build up. Per the manager it is glue from when the floors were redone.
- Observed black build up on the wall behind the hand sink near the walk ins and in the cafeteria dish area. Clean the walls.
