- Observed brown sugar in a large bulk food container without a label. All bulk foods, unless unmistakable, must be labeled with a common name to prevent misuse. Label all bulk foods.
- Observed large dough roller with a large crack on handle and through the body of the roller. When food contact equipment becomes damaged, it is important to repair or replace to ensure proper cleaning. Manager discarded dough roller at time of inspection.
- Observed peeling/rusting shelves in cupboard used to store cleaned and sanitized bowls and plates. Repaint, reseal, or replace to ensure food contact surfaces are smooth and easily cleanable.
- Observed tape and labels on the outside of food contact surfaces that have already been washed, rinsed, and sanitized. Ensure that individuals are removing all debris from outside of food contact surfaces before cleaning.
- Observed wet wiping cloth stored in sink over night. All wet wiping cloths must be laundered when dirtied or stored in sanitizing solution between uses.
- Observed build up of debris on the bottom of multiple refrigerators. Per teacher, the refrigerators were supposed to be cleaned and she was aware of them not being done. All food contact surfaces must be cleaned as routinely as necessary.
- Observed cracked/damaged floor tiles in front of row of refrigerators and in front of back exit door. Floors must be maintained in good repair, smooth, and easily cleanable. Repair the floors.
- Observed build up of debris on floor underneath 3 compartment sink. Clean as routinely as necessary.
- Observed damaged ceiling tile near fume hood where a large white pipe exits. Ceilings must be maintained in good repair, smooth, and easily cleanable. Repair.
- Observed peeling paint on walls behind the food contact surface storage in back storage area. All walls must be maintained in good repair, smooth, and easily cleanable. Repaint, reseal, or repair walls to ensure proper cleaning.

- Observed chafing fuel stored above single service items in the dry storage room. Spoke with the manager and the fuel was moved during the inspection.
- Observed utensils stored with handle down. Utensils should be stored inverted.
- Observed handle of scoop in bulk stored flour. Handle of scoops should be stored out of item to minimize possible contamination.
- Observed some employees without hair restraints. Effective hair restraints such as a hat or a hair net should be worn by employees handling food.
- Observed the seal broken in the True Refrigerator with the red refrigerator label.
- Observed the white microwave with food debris build up. Wash, rinse, and sanitize.
- Observed sink behind the door with a leak. Repair.

- Observed salads in the reach in cooler uncovered. Always cover ready to eat food while in storage.
- Observed worker improperly wash hands. Worker touched handle with bare hand to turn faucet off. Do a retraining on proper hand hygiene. Document what information was gone over and who attended. Submit documentation to TCHD by 03-31-2015.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the tops of cans, the cans of chafing dish fuel in dry storage, and the handsink faucets.
- Observed ceiling tiles with holes in them and ceiling tile outside of the grid in dry storage. Replace ceiling tiles.
