All food temperatures observed were internal temperatures. Manager was notified he successfully completed the Certified Food Manager course. Advised once he has received the certificate, he must bring it to the Hampton Health Department located at 1320 LaSalle Ave to register and submit $10 fee. The Hampton Certificate will be rendered to him at that time and that certificate shall be posted at the restaurant in public view. Report typed at facility but not provided at time of evaluation since peak lunch hour and person-in-charge not available to review/discuss with EHS. EHS to return at a later time with report. FORMAL PERMIT NOT ISSUED. INTERIM PERMIT extended to 12/31/2008.
