Manager was notified he successfully completed the Certified Food Manager course. Advised once he has received the certificate, he must bring it to the Hampton Health Department located at 1320 LaSalle Ave to register and submit $10 fee. The Hampton Certificate will be rendered to him at that time and that certificate shall be posted at the restaurant in public view.
Report typed at facility but not provided at time of evaluation since peak lunch hour and person-in-charge not available to review/discuss with EHS. EHS to return at a later time with report.
FORMAL PERMIT NOT ISSUED. INTERIM PERMIT extended to 12/31/2008. Failure to correct all violations shall result in permit not being issued and facility to remain closed until all violations corrected.

All phfs relocated to other working refrigerators. DO NOT USE THE WALK-IN FOR THE STORAGE OF PHFS--eggs, meat, cheese, dairy--until it is capable of cold holding at or below 41*.
Comments:
CFM candidate advised me that he may not be able to make the CFM course scheduled for tonight. I advised him to contact course director and that he must attend the registered courser or permit shall not be issued resulting in immediate closure once interim permit has expired.
