Observation: Wiping cloths improperly stored between use.
Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

Observation: The sink basin at the hand sink in the kitchen is slow to drain.
Correction: Plumbing systems and components shall be maintained in good repair.
3180 - Physical Facilities - Cleaning Frequency and Restrictions
Observation: Walls behind and floors underneath cooking equipment noted in need of cleaning.
Observation (REPEAT): The floors and the lower half of the walls in the warewashing area are noted in need of cleaning.

This inspection was a follow-up to a complaint inspection conducted on Thursday 05/24/2007. No live roaches were observed during this inspection. A recent pest control receipt was provided by the PIC. A supplemental pest control visit was conducted.

Violations: Comments:
This complaint inspection was conducted due to a complaint of the presence of roaches in the facility. Live roaches were observed in two locations in the facility and dead roaches were observed as well. No evidence was observed of contamination of the food by the roaches. Correct all critical violations today. Please provide recent pest control receipts to the Health Department. Schedule a visit from your pest control company and call for a follow-up inspection with-in 24 hours.

March 22, 2007 (Complaint)

Comments:
A complaint was received on Thursday 03/22/2007 that multiple live roaches were observed in the restaurant on Monday March 12, 2007. No live roaches were observed during the inspection. One dead roach was observed on the floor behind a cooler in the rear of the facility.
The facility receives monthly pest control service. According to documentation furnished by the manager of facility the three most recent applications occurred on 01/19/2007, 02/16/2007 and 03/16/2007. According to notes written by the pest control technician on the invoice dated 03/16/2007 roaches were observed. No unusual activity was noted by the pest control technician. The manager stated that if he observes pest activity he calls the pest control company for a supplementary visit. The facility manager was advised to continue this practice.
The results of the complaint investigation were inconclusive.

0830-The prepared ready-to-eat (RTE) foods in the refrigeration units are not properly dated for disposition.
0830-The ready-to-eat commercially processed foods in the freezer are not properly dated for disposition after opening.
1770 A-The following equipment food-contact surfaces were observed soiled to sight and touch: the can opener blade.
The following non-critical hazards have been corrected since the previous inspection:
0240-Employees observed working in the food service area without proper hair restraints.
0480- Unlabeled food containers. There is a white powder stored in an unlabeled container.
0530-Cooked potatoes are stored in and covered by ice for cooling purposes.
0550-Dispensing utensils improperly stored between uses.
0570-Wiping cloths improperly stored between uses.
1800- The nonfood contact surface of the ice cream cooler has accumulations of grime and debris.
1860-A distinct, separate water rinse after washing and before sanitizing of utensils and equipment was not observed.
2000-Equipment and single-use items were found stored on the floor.
2000-Clean single-use and single service containers were observed stored with the food-contact surfaces facing upward.
2350 ii- Plumbing connections under the mop sink are leaking.
2350 ii- The faucet at the mop sink is leaking.
3045- A sign or poster that notifies food employees to wash their hands is not provided at the handwashing lavatory in the female restroom.
3090-The ventilation fan in the female restroom is not working.
3170-Walls in the dishwashing area are in a state of disrepair.
3170-The door knob is missing from the employee restroom door.
3170-Several floor tiles near the mop sink are broken or missing.
3170-Multiple floor tiles behind the front bar are cracked or missing.
3180-Walls noted in need of cleaning.

October 25, 2005 (Complaint)

Comments:
Received a complaint that one of the employees touched cherries with bare hands when serving ice cream and that another employee was eating soup behind the front counter. Informed the person-in-charge (PIC) at the time of my inspection that employees may not touch ready-to-eat foods with bare hands (must use single-use gloves, tongs, deli tissue or other suitable utensils). Also informed PIC that employees must eat in a designated area where contamination of food, equipment and food contact surfaces is prevented. PIC passed the VDH Foodservice Demonstration of Knowledge quiz.
