• 0550 - Repeat Dispensing utensils improperly stored between uses. Ice scoop stored with the handle on the food.
  Store in-use utensils or dispensing utensils in the following manners: In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed.
• 0610 - Food stored on the floor or food stored less than 6" above the floor.
  Elevate food storage onto approved shelving with minimum 6" legs or casters.
• 1150 - The nonfood contact surface of the cabinet doors is not designed or constructed to be easily cleanable.

  Properly wash, rinse and sanitize the soda dispensing nozzles.
• 2000 - Repeat Observed a bag of nacho containers on the floor of the dry storage room right next to a mouse trap.
  Store single service items on approved shelving with minimum 6" legs or casters. (Correct today).
• 2000 - Repeat Boxes of cups and lids on the floor in upstairs hallway.
  Elevate single service items onto approved shelving with minimum 6" legs or casters.
• 2830 - Floor wall juncture in men's restroom is not coved and closed to no larger than 1/32 inch space.
  Cove floor wall juncture to no larger than a 1/32 inch space.
• 3045 - Repeat A sign or poster that notifies food employees to wash their hands is not provided in the restrooms.
  Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

• 0550 - Dispensing utensils improperly stored between uses: handle of ice scoop was in the ice.

• 0610 - Repeat Food stored on the floor in the dry food storage room upstairs.
  Elevate food storage onto approved shelving with minimum 6" legs or casters.
• 2000 - Repeat Single service items (boxes of cups and lids) stored on the floor in the dry food storage room upstairs and in the hallway upstairs.
  Store single service items on approved shelving at least 6" off the floor.
• 2810 - Floor or floor covering in dry food storage room upstairs is not smooth and easily cleanable.
  Must keep food and single service items elevated on approved shelving at least 6" off the floor. If these items are not kept off the floor, the floor will have to be finished to be smooth and easily cleanable.
• 3045 - A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees.
  Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.
• 3340 1 - Critical Containers of buttery topping stored on the floor of dry food storage room next to containers of cleaning chemicals (Pinesol and Windex). Salt shaker of salt stored with cleaning chemicals on shelf behind service counter.
  CORRECTED DURING INSPECTION. Containers of cleaning chemicals must be separated by spacing or partitioning to prevent contamination of food, equipment, utensils, linens or single service items.
• 1780 - Critical There is an accumulation of mold on the inside surface of beverage dispensing nozzles.
  CORRECTED DURING INSPECTION. Clean and sanitize the beverage dispensing nozzles daily.
• 3170 - There are broken and/or missing floor tiles under the hot water heater and in front of the ice machine.
  Must keep boxes of soda for beverage dispensing machines off the floor ( elevated a minimum of 6" onto approved shelving) in this room. If food and/or single service items are not kept off the floor, then the floor will have to be repaired to be smooth and easily cleanable.

July 21, 2003 (Routine)

• 0610 - Food stored in a location where it is subject to splash, dust or other contamination. An open bin of popcorn was stored right next to the handwash sink at the front counter.
  Store food where it is not exposed to splash, dust, or other contamination to prevent contamination.
• 3020 - Soap was not provided at the hand washing lavatories behind the front counter and in the upstairs restrooms.
  Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
• 2000 - Ice scoop was stored on the drain rack of the beverage dispensing machine.
  Store the ice scoop in a clean,protected location where it is not subject to splash and other contamination.
• 2000 - Single service items were stored on the floor of the dry storage room upstairs.
  Elevate single service items at least 6" off the floor onto approved shelving.
• 3350 - Unnecessary poisonous or toxic materials found in the food establishment. Several cans of paint were stored in the room with the ice machine.
  Remove unnecessary poisonous or toxic materials.
• 2860 - There were some missing ceiling tiles and a loose vent cover above the ice machine.
  Attach ceiling covering so it is easily cleanable.
• 3170 - One of the lights in the dry storage room upstairs is not working.
  Repair or replace the light.
