Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria. The employees discarded the lettuce.
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Several foods in the 2-door and 1-door units across from the grill and stove are at improper cold holding temperatures.
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Employees moved food to the walk-in refrigerator.
Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site) Observation: The prepared ready-to-eat (RTE) lasagna in the refrigeration unit is not properly dated for disposition.
Correction: Mark the name and "consume by" date on food containers of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Harmful bacteria continue to grow even at refrigeration temperatures

Risk Factor
The Person-in-charge (PIC) is knowledgeable regarding employee health. Three employees are certified in food safety management. Good date marking and cooling are observed. The (new) dish machine operates properly and food contact surfaces appear clean and dry. The violations were quickly corrected. We discussed: cooling procedures
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Several foods in the sandwich prep unit are above 41°F.
Correction: Relocate food to a refrigeration unit that is capable of quickly coolding the potentially hazardous food to 41°F or below. Employees immediately moved the food to other refrigeration which was operating properly.
Critical: Toxics - Restricted Use Pesticides - Criteria* (corrected on site) Observation: Black Flag Flying Insect Spray is stored in near the bag-in-box sodas. The owners indicate it is only used outside.
Correction: Store restricted pesticides outside of the food storage area. Inside, use only pesticides which are labelled for use in a food preparation area. Follow all instructions, which may include application only by certified Pest Control Operators (PCO). The pesticides were removed.
