- 3045 - A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees
- 1770 - Critical Observed accumulations of grime and debris on the following food contact surfaces: interior of ice machine and interior of refrigerator.
- 1570 - Hood filter over cook line were observed in a condition that prevents necessary maintenance and easy cleaning. Gasket on ice machine was in poor repair.
- 3080 - Less than 50 foot candles of light was noted over cook line, light was burned out.
- 1960 - Plates were found stacked while wet after cleaning and chemical sanitization.
- 1060 - The nonfood contact surface of the linen where glasses displayed at buffet is not corrosion resistant, nonabsorbent, and/or smooth.
- 3090 - No ventilation in employee bathrooms.
- 2000 - Repeat Clean serving bowls were observed stored with the food-contact surface facing upward. Knives were stored between equipment.
