Each licensed regular restaurant and retail food processing facility conducting meal service activities in the state of Wisconsin must have at
least one manager or operator who is certified in food protection practices. (1) A new food establishment (regular restaurant or a retail food
processor) or a food establishment under going a change of operator after February 1, 2001 shall have a certified food manager within 90 days of
the initial day of operation or provide documentation that they are scheduled within three (3) months to take and pass an approved examination
as specified. (WI State Statutes Chapter 254.71, and section 12-201.11, HFS 196, Restaurants, Wisconsin Administrative Code). Certification
requires that the restaurant permit holder or manager pass one of four state approved exams which tests basic knowledge of food protection
practices. The certified person may not satisfy the requirement for more than one facility. Prepackaged-off premise, temporary and "special
organizations serving meals" restaurant operations and food establishments only processing non-potentially hazardous food or not processing are
exempt from this requirement. Certification involves a) taking and passing an approved exam and b) applying for state certification. After the
exam is taken and passed, the individual must apply for state certification by submitting all of the following: Proof of passing one of the
exams with an official written statement/document from the testing organization; a check or money order for $10.00 made out to the Department of
Health and Family Services; and a completed application. The "Food Manager's Certificate" expires five years after the date of issue, and may
be renewed if the certified manager successfully completes a recertification training course. You may obtain information about courses and
exams from the following organizations: Wisconsin Restaurant Association, 608-270-9950; Tavern League of Wisconsin, 608-270-8591; Waukesha

* Okay to do food order, will arrive on 11/8/10, no food prep until after followup.
* Once all approvals are obtained from other inspectors, okay to open to public.
* Discussed food code fact sheets including: barehand contact, employee reporting agreement, handwashing, cooling, re-heating, glove use,
employee hygiene, etc.
Planning on opening 11/9/10. Will follow-up on the following on 11/ 8/ 10:
* Dish machine to be installed, waiting on tiles to dry.
* Ecolab to come out to hook up sanitizer at sinks and dish machine, will check concentration.
* Food prep sink to be installed in same tile area.
* Final cleaning / organizing prior to food prep and opening.

At time of follow-up inspection, food prep sink installed, sanitizer hooked up to sinks (Ecolab quat 200ppm, test strips avaialble, have water
turned down to lukewarm), Dish machineout of service for time being due to plumbing issues with grease interceptor.
Building inspector has given "verbal" okay, waiting on final plumbing inspection. Tonight is "soft opening" with friends and family -
invitation only. Okay to do food prep once final cleaning / sanitizing of floor / prpe surfaces. From my end warewash / handwash is
available.
Okay to open tonight, per building inspector "okay". Please call to keep me posted as to final approval from plumbing inspector and so that I
cna check dish machine prior to opening to the public.
If more work must be done in kitchen for plumbing, no food prep until cleaning / sanitizing process repeated.
Remember no open to the public until all approvals are obtained.
11/10/10:
Building inspector has issued occupancy permit, plumbing for dish machine still needs to be fixed and final approval obtained form plumbing
inspector. Establishment plans to be closed on Mondays so plumbing work is to be done on 11/15 during closure so no food prep will be done at
that time.
From restaurant licensing standpoint it is okay to release license as sinks are available for warewashing and everyting else is done. Please
call once dish machine is up and running so that we can check sanitizer concentration.
Opening to the public on 11/11/10.
