Critical: DATE MARKING - DISPOSITION (corrected on site) Observation: Potato salad, coleslaw, and prime rib in walk in cooler has exceeded its date mark or were not provided with a date mark. Food employee stated potato salad was to be discarded (and was not provided with a date mark), prime rib and coleslaw were made the previous days and were properly date marked during inspection.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked.
Critical: SEWAGE - BACKFLOW PREVENTION Observation: A direct connection exists between the wastewater system and a drain from the two compartment sinks used for food preparation. Both compartments of sink were full of (thawing) fish during inspection.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Remove the direct connection.
WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES Observation: A Chlorine test kit is not available for checking sanitizer concentrations.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
HANDWASHING CLEANSER AVAILABILITY (corrected on site) Observation: Most soap dispensers at food employee handwashing sinks were broken. As a temporary fix, hand pump bottles of hand soap were replaced at handwashing sinks until dispensers are repaired.
Correction: Provide hand soap at handwashing sink to facilitate proper handwashing.
THAWING (corrected on site) Observation: Frozen fish was thawing in two compartment sinks in water without a constant flow of water to agitate and float off particles. Food employee moved half the fish into the walk in cooler for thawing under refrigeration and the rest was placed into a container to be properly thawed under constant flow of water. Fish had just started thawing at the time of inspection.
Correction: Adjust procedures or methods to properly thaw foods.

Critical: RTE/PHF-TCS PROPER DATE MARKING Observation: Cut/sliced tomatoes, cut leafy greens, cut melons and opened gallons of milk are not date marked. These items are new to requiring date marking. If held for longer than 24 hours provide label and date for items listed above. All but melons were onsite today and not dated. Corrected on site.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
CONSUMER ADVISORY Observation: Consumer advisory on menu is missing asterisks at the food items available to be served undercooked and the asterisk matching the advisory. This has been written 3 consecutive times and therefore should require a re-inspection. Last inspection no advisory existed for the breakfast menu. This time it needs to be updated so no re-inspection will be done or charged. Next inspection it will if not corrected.
Correction: Provide asterisk or designation next to the advisory and matching the asterisk for each food items able to be served undercooked.
POSTING OF CERTIFICATE (repeated violation) Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Post an original State of Wisconsin food manager's certificate. Two certificates are posted but neither are the state certificates. Both posted certificates are on the database for Health Space.
