  • Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER
    Observation: Raw ground beef is stored above raw whole pork in the sliding glass door cooler in the ktichen.
    Correction: Prevent nesting of raw animal foods and store raw animal foods in order of decending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients. Move ground beef whole below pork since the pork has a lower minimum cook temperature. If raw chicken is stored in this cooler the chicken must go below all other animal species.
  • WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER (corrected on site)
    Observation: There is no detectable level of sanitizer in the wiping cloth solution.
    Correction: Cloths used for wiping counters shall be stored in a sanitizing solution. Operator refilled bucket to 100 ppm.
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION (repeated violation)
    Observation: Dish racks are stored on the floor.
    Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off at least six inches off the floor.
  • HANDWASHING AIDS AND DEVICES, USE RESTRICTION (repeated violation)
    Observation: Soap and single-use toweling is provided at food preparation sink (the right side sink with air gap underneath.) Operator is using the left-side sink for handwashing and proper handwashing procedure.
    Correction: Remove handwashing aids and devices at unapproved sink and prohibit handwashing at this sink. Recommended paper towel be mounted to metal storage shelving and soap be mounted to rack with hardware or to wall with adhesive. Holes do NOT need to be drilled in the masonry wall behind the fiberglass since the soap does not need to be mounted at all, and may be a counter-top style hand pump.

    Observation: Food employee observed improperly washing hands by dipping hands in right sink compartment of standing, soapy, soiled water then drying hands on reused cloth towel hanging from clothing.
    Correction: Train food employees in proper handwashing techniques. Use left sink compartment only for handwashing. Water must flow past hands and down the drain. Towels may not be reused for drying hands. Correct By: 27-Mar-2015
  • Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS (corrected on site) (repeated violation)
    Observation: There are ready-to-eat tomatoes and milk stored under raw shell eggs and raw fish in the model AT56B 2 glass door cooler in the kitchen.
    Correction: Fish moved out of cooler to prep area in kitchen and raw shell eggs moved to bottom shelf. Vegetables moved to top shelf.
  • Critical: DATE MARKING - DISPOSITION (corrected on site)
    Observation: Cole slaw dated 3/20 in the True T-23 1 door cooler has exceeded 7 days since its date mark or is date marked incorrectly.
    Correction: Cole slaw date corrected by operator to 3/25. Refrigeration prevents food from becoming a hazard by significantly slowing the growth of most microbes. The growth of some bacteria, such as Listeria monocytogenes, is significantly slowed but not stopped by refrigeration. Over a period of time, this and similar organisms may increase their risk to public health in ready-to-eat foods.
  • Critical: SEWAGE - BACKFLOW PREVENTION
    Observation: An air break exists between the sewage system and a drain from food preparation sink. The lowest point in the drain pipe is even or below the flood rim of the receiving pipe.
    Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Provide a full air gap with vertical separation. Recommended a larger receptor be used if splashing is a concern. Correct By: 27-Apr-2015
  • DEMONSTRATION OF KNOWLEDGE - EMPLOYEE HEALTH
    Observation: Person in charge is unable to demonstate knowledge regarding employee illness signs and symptoms and/or reporting requirements required either by the person in charge or employees.

  • NON-CONTINUOUS COOKING OF RAW ANIMAL FOODS - WRITTEN PLAN (repeated violation)
    Observation: The non-continuous cook process has is not being followed. Pork sausage patties in the Puffer-Hubbard 4 door cooler which are partially cooked are not labeled.
    Correction: Label raw animal food which is partially cooked as partially cooked according to plan approved by regulatory authority. Repeat violation of non-continuous cooking will result in revocation of approval and reinspection for 3rd consecutive repeat violation. Correct immediately.
  • LABELING - FOOD STORAGE CONTAINERS LABELED WITH COMMON NAME
    Observation: Powdered sugar in container under dishwasher, flour, and other foods are stored in unlabeled containers.
    Correction: Store all foods that are removed from their original packages in containers labeled with the common name of the food or ingredient. Correct By: 27-Mar-2015

    Observation: Ice cream scoops are improperly stored in standing water.
    Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F. Correct immediately.
