  • Critical: PREVENTING CONTAMINATION FROM HANDS - NO BARE HAND CONTACT WITH RTE-FOODS (corrected on site)
    Observation: Employee observed handling bread buns with bare hands.
    Correction: Employees shall use single-use gloves, utensils, bakery papers or other approved means when handling ready-to-eat foods. Employee spoke with inspector about alternatives and decided to use gloves to handle ready-to-eat foods.
  • Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS (corrected on site)
    Observation: There was raw beef fillets and steak stored over bread buns, onions, and other ready to eat foods in the walk-in cooler.
    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination. Raw foods were moved under the ready to eat foods during the inspection.
  • Critical: PHF/TCS, COLD HOLDING (corrected on site)
    Observation: Potato salad, coleslaw, and fruit salad were observed on counter at temperatures of 67.1, 66.8, and 57.6°F.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. Foods were moved to the walk-in cooler during the inspection.
  • Critical: FOOD CONTACT SURFACES - SANITIZED BEFORE USE AFTER CLEANING (corrected on site)
    Observation: Utensils and plates were observed to not be sanitized after cleaning. Test of dishwash machine found the sanitizer level at 0ppm during the inspection and sanitizer bottle empty.
    Correction: Clean and sanitize all utensils and equipment before contact with food. New sanitizer bottle was put on the machine and unit was primed. Sanitizer concentration was found at 100ppm.

    Observation: The consumer advisory is provided on the menu but is missing a symbol or asterisk next to it and each food made to order.
    Correction: Provide an asterisk or symbol next to the consumer advisory and each corresponding animal food served in a raw or undercooked condition. Correct By: 26-Aug-2015

    Observation: Dressings and lettuce were observed to be unwrapped or uncovered in the walk-in cooler.

    Observation: Butter, eggs, raw chicken and other foods items were observed stored on the floor in the walk-in cooler.

  • THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE
    Observation: The walk-in and reach-in coolers were not equipped with an integral or permanently affixed temperature measuring device.
    Correction: Equipment used for hot and cold holding shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device. Correct By: 21-Aug-2015

    Observation: The walk-in cooler door gasket is not maintained in good repair. Observed duct tape to be holding areas of the gasket.

    Observation: The cutting board located in the kitchen was observed heavily scored and scratched.

  • NON-FOOD CONTACT SURFACES - CLEAN (repeated violation)
    Observation: Non-food contact surfaces of the outside front and side of the GE 1 door reach in freezer and the floor under the grill were observed soiled with debris and food residue. The condensor in the walk-in cooler was observed to have accumulations of dust and mildew.
    Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. Correct By: 21-Aug-2015
  • HANDWASHING SINKS, INSTALLATION - NON-HAND OPERATED (repeated violation)
    Observation: Handwashing sink is not non-hand operated (has small knobs) and is being operated incorrectly. The current handwashing sink in the kitchen is being used as a handwash sink, vegetable wash sink, and dishwashing.
    Correction: Employee handwashing sinks shall be non hand operated and if metered shall provide a flow of water without the need for reactiviation for at least 15 seconds. Ensure all vegetables are held above the flood level of the sink when washing. Ensure proper handwashing is done at all appropriate times and the sink is accessible. Correct By: 13-Aug-2015
  • WALL AND CEILING COVERINGS AND COATINGS
    Observation: The ceiling above the soda boxes and walk-in cooler is currently covered with cardboard.
    Correction: Provide wall and ceiling coverings that are smooth, easily cleanable and nonabsorbant. Correct By: 26-Aug-2015

    Observation: Wiping cloth used for wiping counters noted sitting on counter not stored in sanitizer solution.
    Correction: Store wiping cloths in sanitizer soolution of 200 ppm chlorine when not in use. Correct By: 22-Mar-2014

    Observation: Ribs are being improperly thawed by sitting on drainboard of sink over dishwasher.
    Correction: Adjust procedures or methods to properly thaw foods. Thaw foods on lower shelf on a cooling unit or other method to prevent food bourne illness. Correct By: 19-Mar-2014

    Observation: Ribs are being thawed on drainboard of the one sink in the kitchen/prep area currently used for pre-wash, hand wash, prep sink and the mop sink. Provide and have in place a sink used solely for the purpose of food preparation.
    Correction: A food preparation sink shall be provided for food preparation and provide an air gap between the sink drain and the sewer. Existing sanitizing compartment of bar 4 compartment sink may be used for food prep if properly air gapped and an insert is used to prevent cross-contamination of adjacent sinks. Use the sanitizing compartment immediately and install a separate food prep sink within 90 days. Correct By: 15-Jun-2014

    Observation: Fire extinguishers and ANSUL system were last inspected January 2013, over 12 months ago. Carbon dioxide (CO2) tank was not secured (secured duribng inspection).
