  • Critical: PREVENTING CONTAMINATION FROM HANDS - NO BARE HAND CONTACT WITH RTE-FOODS (corrected on site)
    Observation: Employee observed handling ready to eat sandwich with bare hands.
    Correction: Employee was instructed by health inspector and food manager to wash hands, put on disposable gloves, and not to handle ready to eat foods with bare hands. Sandwich was not served to public and discarded.
  • Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS (corrected on site)
    Observation: There are ready-to-eat foods (cooked bacon and sausage, chocolate chips and other foods) stored under or adjacent to raw foods (eggs, chicken and beef) in multiple coolers. The raw and/or RTE foods were reorganized and separated during the inspection.
    Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.
  • Critical: PHF/TCS, COLD HOLDING (corrected on site)
    Observation: Raw shelled eggs were observed sitting on countertop at 78°F - eggs were there approximately 2hrs. These items were immediately placed in walkin cooler.
    Correction: Maintain cold potentially hazardous foods at or below 41°F
  • Critical: RTE/PHF-TCS PROPER DATE MARKING (corrected on site) (repeated violation)
    Observation: Plates of cooked bacon in the walkin cooler are not date marked. Manager indicated these were made today and they would be used within 2 days.
    Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

    Observation: No air gap provided on ice machine in kitchen area.
    Correction: Provide an air gap on water supply side to protect water supply.
  • CONSUMER ADVISORY
    Observation: No consumer advisory is provided on menu indicating consumption of raw or undercooked foods may increase risk of foodborne illnesses..

    Correction: Maintain food contact surfaces in a clean condition.

    Observation: Hot water temperature at kitchen handwashing sink is at [131 ]°F.
    Correction: Water temperature at a handwashing sink shall be at least 85°F and no more than 110°F.
  • USING A HANDWASHING SINK - ACCESSIBLE (corrected on site)
    Observation: Kitchen handwashing sink observed to be blocked/obstructed by large waste container during inspection and is unavailable for proper handwashing.
    Correction: Keep handwashing sink free of obstructions and in working condition at all times to allow for frequent handwashing.
  • POSTING OF CERTIFICATE (repeated violation)
    Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
    Correction: Provide & post an original State of Wisconsin food manager's certificate.

    Observation: Multiple foods in multiple coolers were observed to be unwrapped or uncovered.
    Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.

    Observation: Ice scoop handle within ice bin adjacent to front counter is observed touching the ice.
    Correction: Store in-use utensils in the food item with handle extended or on a cleanable, smoothe and durable surface.
  • FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
    Observation: Potato's, flour and cases of food are observed stored on the floor in the dry storage areas and walkin freezer.
    Correction: Store all food items 6 inches above the floor.
  • FOOD CONTACT SURFACES - MICROWAVE CLEANED (corrected on site)
    Observation: Interior door of microwave is soiled.
    Correction: Clean and maintain microwave.
  • UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
    Observation: Dishwasher racks are being improperly stored in on the flooring near the kitchen warewashing area.
    Correction: Clean equipment and utensils shall be stored in a clean dry location, not exposed to contamination and off the floor.
  • WALL AND CEILING COVERINGS AND COATINGS
    Observation: Wall covering is above shelving in back dry storage area is damaged (bare wood exposed) and is no longer easily cleanable.
    Correction: Provide wall and ceiling coverings that are smooth, easily cleanable and nonabsorbant.
  • LIGHT BULBS - PROTECTIVE SHIELDING
    Observation: Lights located above food prep area are not shatter resistant or protected.
    Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
  • INSECT CONTROL DEVICES, DESIGN AND INSTALLATION - IMPROPERLY LOCATED (corrected on site)
    Observation: The insect control devices (fly paper) are located over warewashing area in kitchen.
    Correction: Remove or relocate this device so that it is not over any food prep area.
  • INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
    Observation: There is a gap between the back outer door and the floor which could allow the entrance of insects or rodents.
    Correction: Install effective door sweep on base of door.
  • INSECTS AND RODENTS - OUTER OPENINGS - SCREENING
    Observation: The back screen door is in disrepair and not effectively covering the door opening, thereby potentially allowing the entry of insects or rodents.
    Correction: Repair or replace screen door to prevent entrance of rodents and insects.
  • HANDWASHING SIGNAGE (corrected on site) (repeated violation)
    Observation: No handwashing signage provided at kitchen handwashing sink.
    Correction: Provide handwashing signage at all handwashing sinks used by food employees.
