An advisory for consuming raw or undercooked products was not provided on the menu.
An employee was observed handling sushi, then dirty dishes, then sushi again without changing gloves and washing hands in between the tasks.
Raw chicken (46�°F) and raw steak (45�°) were above 41�°F at the cold-top refrigerator across from the fryers. A container of lettuce was 63�°F next to the cold-top refrigerator. See voluntary condemnation agreement.
The dish machines final sanitization rinse measured less than 50 ppm chlorine residual.
Paper towels were not provided at the hand sink behind the sushi bar.

An unapproved employee twist-top beverage was observed in the kitchen area.
