Holes in the walls were present in the back boys restroom. Several ceiling tiles throughout the facility were stained. Several lights throughout the facility were not functioning. Several floor tiles throughout the facility were chipped.
Several toy storage shelves throughout the facility were chipped. Tape on the ball pit was not smooth and easily cleanable.
An employee was observed putting on gloves without first washing their hands.
The interior of the kitchen cabinets were soiled with food debris. The base coving in the kitchen was torn.
