All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. ALL COOLERS WITH POTENTIALLY HAZARDOUS FOODS (EX. MILK, EGGS, PREPACKAGED LUNCHMEAT ETC) MUST MAINTAIN COLD HOLDING TEMPERATURE OF 40F OR BELOW.
All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO HOT RUNNING WATER ON PREMISES AT THIS TIME. INSTRUCTED TO PROVIDE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. INSTRUCTED TO PROVIDE LICENSED PEST CONTROL SERVICING PREMISES WITH PEST CONTROL LOG BOOK (SERVICE AGREEMENT, SERVICE REPORT, CHEMICAL USED ETC).---------MUST RODENT PROOFED BOTTOM OF FRONT DOOR.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. INSTRUCTED TO PROVIDE GARBAGE DISPOSAL COMPANY SERVICING BUSINESS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN FLOORS INSIDE BOILER ROOM.---------MUST PROVIDE FLOOR BASEBOARD AROUND REAR STORAGE AREA.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO REMOVE PEELING PAINT ON CEILING INSIDE WASHROOM AND REPAINT.--------MUST SEAL HOLES ON WALLS IN REAR AND IN DISPLAY AREA.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. LIGHTS MUST BE SHIELDED WITH ENDCAPS ABOVE FRONT AND IN REAR AREA.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO INSTALL VENTILATION INSIDE WASHROOM.-------MUST INSTALL MOP SINK WITH AN ATTACHED BACK FLOW PREVENTER DEVICE OR VACUUM BREAKER AT MOP SINK FAUCET.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. TOILET ROOM DOOR MUST BE SELF CLOSING. INSTRUCTED TO INSTALL SELF CLOSING DEVICE.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. INSTRUCTED TO SEAL OR PAINT RAW WOOD DOOR AND DOOR FRAMES IN REAR AREA.
