This must be a relatively recent development. Both cities are separate and they are unique in and of themselves. As far as I could tell, these differences were always respected.
I have heard this "Milw suburb crap" mainly within the Chicago media, radio and print newspapers especially from Jag Mariotti.

Well, as a native of Illinois and the Chicago area, I never heard such a thing. Absolutely ridiculous.

Don't forget that Kenosha, Racine, Waukesha, and other communities inundated by the Chicago or Milwaukee sprawl are also bona fide and historically independent cities.

Saying "Milwaukee is a Chicago suburb" is as cutesy and snarky as "Canada is the fifty-first state of the USA." It's often said in the context of a light-hearted rivalry, but may reveal a sort of empire mentality.

Speaking of which, I always have to laugh when I met people from Waukesha that try to tell me that Waukesha isn't a suburb of Milwaukee...lol.

Waukesha has an identity problem. Always pretending they are something they are not.

By 2020, if not 2010, they will be counted as one Metro. To say Milwaukee would be a suburb of Chicago is ridiculous though. That's like saying Fort Worth is a suburb of Dallas, its entirely untrue. Anyway, I'm sure Milwaukee will have to swallow its pride and get use to the term "Chicagoland." At least they get to say hello to the METRA !
