The outside showers I've been in all have like a wood fencing all around them with a concrete floor - only your feet show. But the neatest thing is being able to hear the ocean nearby and look up to see the big blue sky
Oh, yeah, I've seen pictures of those in some "rustic" fancy resorts. They ARE lovely! Never tried one, though.
I was gonna go once to a place near Puerto Vallarta with open cabanas and such showers, but the thought of having to get there on a small water taxi finally made me decide against it...

Not really (unless they're of European descend). There are exceptions, though. On my last vacation I saw one who looked quite like Antonio Banderas!
