Observation: PAPER HAND TOWEL DISPENSER IS EMPTY AT FRONT handwashing sink for hand drying. HAND TOWEL DISPENSER NOT PRESENT AT BACK HANDWASHING SINK.
Correction: STAFF PLACED PAPER TOWEL ROLLS AT SINKS TO DRY HANDS. STAFF HAS ORDER IN FOR CORRECT PAPER TOWELS TO FIT DISPENSER. ORDER SHOULD ARRIVE IN 2 DAYS. STAFF WILL PUT PAPER TOWEL DISPENSER AT BACK SINK. Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.
- EQUIPMENT AND UTENSILS - PROPER ADJUSTMENT
Observation: WALK IN FREEZER HAS LARGE AMOUNT OF FROZEN CONDENSATE BUILT UP ON FLOOR. THE AMORED ELECTRIC CABLE CONTROLLING THE FREEZER IS BROKEN WITH SHARP EDGES RISKING THE ELECTRIC WIRES INSIDE.
Correction: STAFF WILL BE REMOVING CONDENSATE FROM FLOOR AND IS AWARE OF THE POTENTIAL RISK TO ELECTRIC WIRES/FREEZER OPERATION. Equipment components shall be kept intact, tight, and properly adjusted.

- CONTROLLING PESTS - TRAPPING DEVICES
Observation: Presence of fruit flies is found in restaurant food service area. One bulb on fly trap is not lit. Managers use a pest control company to control the fruit flies. Pest control company invoices were checked and fly devices and floor drains are being routinely maintained.
Correction: Eliminate pests or insects from food establishment. It is recommended that business continues to work with the pest control company. More frequent pest control scheduling may be needed. Company will be called to replace fly bulb that is inoperational. Correct By: 24-Nov-2015

Observation: Milk and other dairy products in the prep cooler and dispensing coolers were found to be at temperatures of 47-59 degrees°F.
Correction: Maintain cold potentially hazardous foods at or below 41°F. Some product disposed of and some product moved to another cooler that is maintaining proper temps. The dispensing units were removed from service.
- Critical: FOOD CONTACT SURFACES - CROSS CONTAMINATION
Observation: Employee placed packaged food on the floor prior to placing the food product in the coolers for product currently being distributed to the public for consumption.
Correction: Clean food contact surfaces anytime contamination has occurred.

Observation: Quat sanitizing solution is stored on top of the ice making machine.
Correction: Rearrange area so that toxic materials are stored below or away from food, equipment and single service items. The Quat solution was removed from this area while inspector was on site.

Observation: The Person in Charge (PIC) is not performing the following duties: Product cold hold temperatures were not observed for dairy products.
Correction: The person in charge shall provide training to employees regarding proper temperature maintenance.
