3-302.11(A)(2) - --There were two trays of raw steak stored above a tray of raw fish on a rack in the walk in cooler. COS- PIC rearranged the rack so that all products were arranged correctly.
KDA_3_30211A1a - --There was a tray of raw lobster tails stored directly above containers of cut raw peppers and peas in reach in cooler 2 in the prep area. COS- PIC moved the vegetables to a different cooler. There were two trays of raw steak stored directly above a bag of raw broccoli florets on a rack in the walk in cooler. COS- PIC rearranged the rack so that all products were arranged correctly.
KDA_3_30211A1b - --There was a rack of raw chicken stored directly above a box of RTE butter in the upright reach in cooler in the prep area. COS- PIC moved the butter to a different cooler.
KDA_3_50118A1 - --There was a container of RTE brown rice in the glass reach in cooler in the sushi area that was date marked 1/10. PIC stated 1/10 was the prep date. COS- PIC voluntarily discarded the rice.
KDA_4_50111A - --The floor is broken and coming unattached inside the walk in cooler. PIC stated the item is set to have maintenance done.

KDA_6_501110B - --There were two employee jackets hanging on the dry storage racks in front of the walk in cooler in the back.
KDA_6_50116 - --There was a wet mop with a handle stored in the mop bucket not allowed to properly air dry.
KDA_7_10211 - --There was a working spray bottle of purple liquid stored under the prep counter in the sushi area without a common name listed on the label. PIC identified the product as degreaser. COS- PIC correctly labeled.
KDA_Defacto - --Chlorine measured at 50ppm in dishmachine in back, test strips provided. There was no Chlorine reading in the bar dishmachine. The machine was not in use by staff. Recommended maintenancing before use. --Discussed using time as a public health control for the sushi (rice) preparation. No rice was being held at room temperature during inspection. No HACCP or variance in place for use of vinegar to maintain correct acidity. Recommended procedure and log for time control, will be implementing immediately per PIC. --miso soup 156 (hot hold in sushi prep area) rice 149 (rice warmer in sushi) --rice 41 (glass reach in cooler sushi) raw tuna 40 (reach in cooler 1 sushi) shrimp 39 (reach in cooler 2) raw fish 40 (sushi display cooler) rice 39 (walk in cooler) raw chicken 36 (upright reach in cooler back prep) raw lobster 37 (reach in cooler 2 prep) shredded cabbage 42 (reach in cooler 3 prep) --There is a pest control company contracted, no paperwork provided. No activity.
