Five Sixty would be Dallas' most famous example. It's the restaurant in Reunion Tower. (Or, as it's more commonly known as, The Big Ball.) It also rotates.

There may be other examples of restaurants on some of the higher floors in some of the skyscrapers, but I don't know them off hand.

San Francisco has the Nordstrums restaurant at the top of the Westfield Mall (on top of the Powell St. BART)

How about the roof of the W hotel? Ghost Bar (I think?).

It's a bar, so not really included in this discussion. It is on the roof though, so maybe it's close enough. Either way, good call. Top floor!

LA, NYC, and Vegas have plenty. SF has a bunch as well (Grand Hyatt, Top of the Mark, etc). The circular, revolving restaurant at the Hyatt Regency was awesome, but they closed it and turned it into their top-tier rewards program members' club floor, which was phenomenally stupid business sense.

sundial (rotating, indoor) atop westin peachtree in downtown
nikolai's roof (stationary, indoor) atop hilton in downtown
polaris atop the hyatt regency downtown - not used currently, as (22 story) building buried in the middle of a bunch of otherwise similar, boring midrises...

skylounge (outdoor) atop glenn hotel in downtown
rooftop 866 (outdoor) atop renaissance (previously palomar) in midtown
