I happen to teach etiquette classes. There are ideals and then there is real life . . . and referring to a Senator as Ma'am is NOT an infraction of any sort.

It wasn't a lack of respect, it was a failure to follow protocol. This was a formal senate hearing not lunch at the deli. Both he and she should use the proper forms of address on those occasions. It's surprising that a senior military officer made such a faux pas. Secretary Gates probably took him to the woodshed big time after the hearing.
The thing is, it wasn't a "faux pas". It is 100% acceptable to refer to ANY Senator as Sir or Ma'am in any setting, INCLUDING a formal hearing.
In the military you use someone's rank when they are below you and "Sir" or "Ma'am" when addressing a superior. Same with the Queen of England, you address her a "Your Majesty" upon the first address, then "Ma'am" after that. Sen. Boxer obviously has inferiority issues.
Her title is Senator and you will damn well address her correctly when in a Senate hearing. If you can't get that through your neo-con brain then you will damn well be asked to leave the hearing.
Easy for you to say. She is still a twit with a capital Tw. I bet she and Pelosi are sniffin each other out at San Francisco parties. Bowwow!
They will be around for a while so get used to it.
