*4.5/4.6 The pop nozzles were soiled. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch. Pop nozzles were cleaned today.
*5.1 Provide an air gap between the pipe and the floor drain under the preparation sink to prevent possible contamination of food items in the sink.
6.4 Observed the base cove by the newly repaired wall surface in the walk-in cooler was in disrepair. Repair this area to provide a cleanable surface.
R/6.2 Repair the non-functioning light fixtures (to provide lighting as specified in OAC 3717) in the grill and food preparation area. Currently 25-33 foot-candles.
R/4.4 Door seal on the reach-in cooler was damaged. Repair these areas.
4.8 Observed refrigeration racks were stored on the floor in the rear storage area. Store equipment six inches off the floor to facilitate cleaning and to prevent contamination.
4.5 Clean the dust accumulation on the Traulsen freezer condenser.
6.1 Keep the rear door closed to prevent pest entry.
Notes: Currently the Star Hot dog roller was in disrepair. Reportedly a new unit was on order.

*3.4 Chicken salad sandwich was lacking a date mark. Foods shall be dated the day of preparation or opening and shall be used or discarded in 7 calendar days to limit the growth of organisms. This food item was discarded today.
6.4 Observed wall panels in the walk-in cooler contain a gap. Repair this area to provide a cleanable surface. Caulk was purchased for repairs and will be installed in the near future.
R/6.2 Repair the non-functioning light fixtures (to provide lighting as specified in OAC 3717) in the grill and food preparation area, and in the women's restroom.
4.5/4.4 Door seals on the reach-in cooler were soiled and were damaged. Clean and repair these areas.
6.4 Observed the floor/wall junctures in the rear storage area were soiled. Clean this area.
4.8 Observed refrigeration racks and cups were stored on the floor in the rear storage area. Store food and equipment six inches off the floor to facilitate cleaning and to prevent contamination.
4.5 Clean the dust accumulation on the Traulsen freezer condenser, shelves in the walk-in cooler appeared to contain a mold-like growth.
Notes: Operation added a new commercial Amana microwave, and a new commercial hand sink.
