4/24/2012Standard 1COLD HOLDING: Observed the following foods cold holding improperly:
-ground beef awaiting prep on the counter 58°F
-meatballs with sauce in the upright cooler 44°F
Maintain potentially hazardous foods (PHFs) cold holding at 41°F or below in order to limit bacterial growth. Discussed cooling meatballs to 41°F prior to covering with plastic wrap. PIC immediately cooked the meatloaf.
-Observed PIC properly wearing gloves to prevent bare hand contact.
-Observed PIC properly washing hands between tasks and glove changes.
-Observed proper date marking throughout.
-PIC was knowledgeable of proper raw animal food storage and reheating procedures.
-1 accurate digital thermometer was available for use.
  • R/6.4 Continue replacing missing floor tiles under the preparation sink to facilitate cleaning. This violation has been listed on standard inspection reports since April of 2007. Continue to repair this area. This office will monitor this violation during the first standard inspection of the 2012-2013 licensing season, if repairs are not complete a compliance agreement will be created between this office and the above listed operation.
  • 4.4/6.4 Repair the following damaged areas:
  • R/- damaged caulk above the three-vat sink
  • - non-functioning employee restroom
  • - ceiling and wall surface in the hallway restroom area damaged or uncleanable.
  • R/- ceiling tiles were stained in the kitchen. Be advised these tiles must be replaced with cleanable ceiling tiles.
  • Provide a cleanable surface.
  • R/4.5/6.4 Observed mouse droppings in the cabinet with mouse bait. Provide pest control measures, wash and sanitize the interior of these surfaces.
  • R/6.2 Repair the non-functioning light fixtures in the kitchen area, to provide lighting as specified in OAC 3717.
  • 6.1 Repair the screen door to provide a tight -fitting door, to prevent pest entry.
  • 5.4 Provide a covered can in the employee restroom.
  • 6.2 Provide a hand washing sign in the employee restroom to inform employees where to wash hands.
  • 5.1 The toilet seat in the employee restroom is damaged and uncleanable. Repair this area.
  • 6.4 Clean the light shield in the storage room, which contains dead insects.
  • Note: Non-commercial refrigeration unit in the rear storage area contained employees personal items today.

  • C/*4.4 QAC sanitizer in the wiping cloth bucket contained 400ppm. Maintain QAC sanitizer at 200ppm in order to effectively sanitize.

  • 5.4 Keep the dumpster lids closed to prevent pest entry.

  • R/- damaged caulk above the preparation sink, and hand sink
  • R/- damaged wall tile in the employee restroom
  • - ceiling tiles were stained. Be advised these tiles must be replaced with cleanable ceiling tiles.

  • R/4.1/9.0 Observed the operation added a non-commercial refrigeration unit to the operation. Refrigeration unit contains patient
  • Boost drinks, and carbonated beverages. Remove non-commercial grade equipment, and provide commercial grade equipment.
  • R/4.5/6.4 Observed mouse droppings in the cabinets. Provide pest control measures, wash and sanitize the interior of these surfaces.
  • 4.5 Observed the backsplash on the stove top oven unit is soiled and contains food accumulation, and the fan cover in the upright cooler unit is soiled. Clean these areas.
  • 4.4 Repair the non-functioning ice machine. Equipment shall be maintained in a state of good repair.

PROTECTION FROM CONTAMINATION: Observed a dented can of corn. Packages of foods shall be in good condition to protect the integrity of the contents and to prevent contamination. Relocate this protect to a separate designated area.
  • R/6.4- Continue replacing missing floor tiles under the preparation sink to facilitate cleaning.
  • 4.4/6.4 Repair the following damaged areas:
  • R/- interior of the utensil drawer is damaged
  • R/- both door seals on the True refrigeration unit
  • R/- damaged wall tile in the employee restroom
  • - hood unit was not properly functioning today.
  • Provide a cleanable surface.
  • 4.5/6.4 Observed mouse droppings in the cabinet under the three vat sink. Provide pest control measures, wash and sanitize the interior of these surfaces including the exterior which are sticky.
  • Note: This inspection was rescheduled form during the week of 6/21/10, to the date listed above.
