MEASURING DEVICES HELD INGREDIENTS WITHIN AND EXPOSED TO POSSIBLE CROSS CONTAMINATION ISSUES.

INSPECTOR FOUND PATIENT'S NUTRITIONAL MIXTURE IN WALK IN COOLER WITH A PREPARATION DATE LISTED AS 9/27/2013. IN ADDITION, LABEL DID NOT INCLUDE A USE-BY DATE. NUTRIENT MIX WAS 6 DAYS BEYOND THE ACCEPTABLE 'USE-BY' DATE AND SHOULD HAVE BEEN DISCARDED NO LATED THAN 10/3/2013. MANAGEMENT DISCARDED ITEM IMMEDIATELY.

STAFF MEMBER HANDLING READY TO EAT FOOD ITEMS, CRACKERS, WITH BARE HAND AND NOT HAVING WASHED OR GLOVED HAND

- A CERTIFIED FOOD MANAGER MUST BE ON DUTY DURING ALL HOURS OF OPERATION. 1) no certified food manager on duty during routine inspection.
- Move the ware wash machine closer to the floor drain or the floor drain closer to the ware wash machine. INSPECTOR WITNESSED THAT DRAIN OF SCULLERY/WARE WASHING MACHINE NOT DRAINING DIRECTLY INTO FLOOR DRAIN. WATER PLUME DRAINING FROM SCULLERY MACHINE WAS PARTIALLY DRAINING ONTO FLOOR IN WAREWASHING ROOM, RESULTING IN STAFF WALKING ON FLOOR WET WITH GRAY WATER WHILE PERFORMING TASKS. 1) Correct drain line allignment
