  • Observed employees not wearing effective hair restraints.
  • In the walk cooler the silver racks are rusting as well as the reach in cooler on the same side as the fryers. The covers for the motors on the reach in cooler are missing on both sides. There were dirty rags in this area.
  • The 4-door cooler/freezer unit has buildup in the seals and handles. The blue rack in the walk in cooler have buildup on them. The milk crates that have lids in them need cleaned.

  • Observed three pans of raw chicken and a pan of chicken wings out of date. The manager voluntarily discarded the items during the inspection.
  • Observed employee eat food from the line. Employees should only eat in designated areas.
  • Observed exposed insulation in the beer cooler to be accumulating black debris build up. Repair.
  • Observed dishes on the clean dish drying shelves to have food debris build up on them. Wash, rinse, and sanitize dishes. Observed the ice machine to have pink build up in it. Routinely wash, rinse, and sanitize.
  • Observed the reach in coolers by the grill to have food debris build up and rusty racks as pointed out to the kitchen manager. Routinely clean.
  • Observed pans stacked wet. Air dry.
  • Observed the floor around equipment such as the fryer, grill, and the the three compartment sink to have debris build up. Routinely clean.

  • There are still many unused items in the outside storage room. Continue disposing of these items.

  • Observed a container of Strawberries in the walk in cooler and in salad cooler with fuzzy grey growth on them. The two containers were voluntarily thrown away by management during the inspection.
  • Observed spray bottle and squeeze bottle of oil on grill line unlabeled. Label.
  • The following food in cold holding in the salad prep cooler was above 41 F and voluntarily thrown away by the manager during the inspection: Coleslaw 46 F, sliced tomatoes 47 F, pico de gallo 47F, and wild rice 51 F. Alfredo sauce in the drawer cooler under the grill was at 48 F and thrown away at the time of inspection. All other food items in the same coolers were 41F or below.
  • Observed bulk container of Tortilla chips in the back storage prep area uncovered.
  • Observed both salad prep coolers missing covers. Provide covers.
  • Observed a single use dressing cup being used as a scoop. Provide scoop with a handle.
  • Observed broken containers on clean dish shelf.
  • Observed clean containers with old labels still attached to the outside. Replace missing front panel of salad cooler. Replace missing front panel of sundae cooler. Observed missing counter top edge behind bar storage.Observed shelf wrapped in saran wrap in bar cooler.
  • Observed temperature gauge on the dish machine broken. Repair or replace.
  • Observed food build up inside and outside dish machine and dish machine area. Wash, rinse, and sanitize this area.
  • Observed cloths on the floor in the back prep storage room. Wiping cloths should be stored in sanitizing solution between uses.
  • Observed food debris build up inside microwaves and all reach in salad coolers. Observed pink goo build up on the inside of the ice machine. Wash, rinse, and sanitize all areas.
  • Observed the following areas with a build up of debris: bulk rice container, bulk Tide container, beer cooler seal, containers on clean dish drying rack by ice machine, and all salad prep cooler seals. Wash, rinse, and sanitize these areas.
  • Observed single service food items stored on the floor in the outside storage room.
  • Observed open single service items stored with cleaning supplies such as brooms, and mop heads.
  • Observed personal items stored with single service items in the bar area.
  • Observed prep sink with leaking faucet in back prep storage room. The three compartment sink in the kitchen has leaking faucet. Three compartment sink behind bar has leaking faucet handle. Hand sink in the bar has leaking faucet.
  • Observed gap in the roof of the outside storage room with single service items such as single service items. Remove these items from the storage shed.
  • Observed floors throughout facility with debris build up, especially in the following areas: all floor drains, floor behind fryer, and the floor in the walk in freezer.
  • Observed seals along the wash machine station in poor repair. Re-caulk or re-seal.
  • Observed excessive food service items in outside storage room. Spoke to the manager many of the items are no longer in use. Remove these items from facility.
  • Locate and provide cool down documentation to TCHD, Attn: Kim by 10/9/15. Emailed chart.

  • Observed employee touching cooked food with bare hand before it was to be served to customer. Employee washed hands and put gloves on both hands.
  • Observed employee playing with his self phone while wearing gloves and going straight back to preparing food. Employee was asked to put his phone away and washed his hands and put on a new pair of gloves. Conduct a training with staff on proper hand washing and glove use and submit documentation to TCHD, Attn: Kim by 4/7/15.
  • The chemical storage shelf in the back storage area was damaged. Repair or replace it. The 4-door stainless Johnson cooler/freezer in the kitchen and the grill prep cooler area both missing compressor covers, exposing wires and causing them to accumulate debris. Repair them.
  • The dish machine was very dirty. Clean it.
  • The following food contact surfaces had a build up of debris: stainless counters, cooler interiors, walk in cooler shelves, and make tables. Clean and sanitize these areas.
  • The grills, fryers, microwaves, and hot holding areas had a build up of grease and debris. Clean and sanitize them.
  • The outside of Evolution Steamer, outsides of coolers and freezers, mop sink area, dirty side of the dish washer counter/drainboard, cooler compressors, and monitors in kitchen had a build up of debris. Clean and sanitize these areas.
  • The back hand sink by the walk in coolers had no hot water and the bathroom hand sinks only had warm water. Manager checked the hot water heaters. One of the 2 tank-less hot water heaters was not properly functioning. The 3-compartment sink and 2 kitchen hand sinks still had hot water. Repair the hot water heater and contact TCHD as soon as it is working.
  • Repair or replace leaky faucets at bar 3-compartment sink and hand sink, prep sink in back area, and 3-compartment sink in the kitchen.
  • Found build-up of debris on floors. Clean the floors throughout kitchen and prep areas, walk-in coolers and freezer, and under and around equipment..
  • Replace missing ceiling tile outside of beer walk in cooler.
  • The kitchen walls and walk in cooler walls had a build up of debris. Clean the walls.
