- Observed 8 jars of condiments that had black growth on the containers. These items were voluntarily disposed of by the owner at the time of this inspection.
- Observed thermometer at 38 F when in ice bath. Thermometers must be calibrated and maintained at 32 F in ice bath.
- Observed broken seal on prep cooler. Replace. Observed seal on 3 compartment sink and wall that is missing. Reseal. Observed inside door of walk in cooler that has chipping paint. Repaint.
- Facility uses quat sanitizer and does not have any test strips. Provide proper test strips for sanitizer.
- Observed debris accumulation on and in the double oven. Clean and ensure routine cleaning. Observed debris accumulation in white freezer by dessert area. Clean and ensure routine cleaning.
- Observed debris accumulation on the following items/surfaces: All carts in kitchen, Ice machine, Racks and shelving units throughout kitchen and throughout walk in cooler, Cabinet beneath coffee machine in server area, and the seals on the cooling units in kitchen. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed cups stacked together wet. Ensure dishes are air dried before storage.
- Observed food containers stored with the food contact area facing upwards. Store inverted to protect from possible contamination.
- Observed broken floor tiles throughout kitchen. Replace. Observed missing floor pieces with appearance of water damage in server area. Replace.
- Observed debris accumulation on floors throughout facility and in walk in cooler especially beneath shelves and equipment. Clean and ensure routine cleaning.
- Observed debris accumulation on walls and ceilings throughout kitchen and in the walk-in cooler. Clean and ensure routine cleaning.
- Observed pesticide stored next to chemical cleaner. These categories of chemicals must be stored physically separate. The pesticide was moved to a separate area at the time of this inspection.
- Observed refrigeration units at facility that are not in use and are not operational. Repair or remove.
- The facility does not have any temperature charts for cold storage. Temperature charts must be maintained. Facility does not have dates on cool down charts, just months. Ensure proper date labeling is provided for cool down documentation.

- ound open gallon 2% milk in the walk-in cooler. Facility must date label milk when it's open if not used in 24 hours. Also, facility must use a milk dispenser unit for pouring into drinking glass or carton milk.
- Observed kitchen staff without the use of hair restraints. Ensure effective hair restraints are used.
- Observed Seals on several coolers that are beginning to wear. Reseal or replace. Observed small fridge in waitress station that is not operational. Repair or remove.
- Observed dust accumulation on ceiling in kitchen around air vents. Clean and ensure routine cleaning.
- Observed insecticide stored next to cleaning chemicals. Items in these two categories must be stored physically separate. Insecticides were removed by staff at time of inspection.
- Observed sanitizer stored on a shelf with coffee pots and above single use containers. Sanitizer was moved by staff at time of inspection.

- Several reachin coolers do not have thermometers.
- Found several paper bags of dry goods uncovered in dry storage area.
- Observed several employees improperly washing hands. The employees touched the handle of the faucet with their hands instead of using a paper towel to turn off the faucet. Also the duration of hand washing was less than 20 seconds. At time of inspection the hygiene issue was discussed with owner. If the facility can not demonstrate good hygiene/handwashing the privilege of a bare hand policy may be revoked. You need to conduct an extensive training going over proper handwashing. You also need to watch every employee (including servers) properly wash their hands. Documentation of this must be submitted to TCHD by 10/24/14.
- Wash rinse and sanitize the following: Microwaves, inside and outside of cooling units, inside and outside of all cabinets and drawers, dishrack shelves, around dish machine, inside and outside ovens, and panini machine.
- Glasses were found in server station and on drying rack that were stacked wet. Trays were found on drying rack that were stacked wet.
- Floors throughout kitchen area had buildup of food and debris.
- Walls throughout kitchen had a buildup of food and debris. Ensure regular cleaning.
- Febreeze was found on shelf with condiments near deep fryer. Paint can was found on shelf with a box of crackers.
